Objective: To investigate the clinical value of CT examination in the diagnosis of benign and malignant thyroid calcification.Methods: 120 patients with thyroid cancer treated in the Second Hospital Of Anhui Medical University and 120 patients with benign nodules were selected.232 patients with thyroid calcification nodules admitted to the Second Hospital Of Anhui Medical University and 176 patients with thyroid calcification nodules admitted in the same period were selected.The results of the study were verified.According to the calcification morphology,it is divided into fine particles and coarse particles calcification;according to the position of calcification in the nodules,it is divided into the center and the edge;statistical fine particle calcification,central and the combination of the two in the benign and malignant nodules,and Statistical analysis.All patients in group A and group B underwent CT examination,and the detection rate of the two groups of patients was calculated.Results: The detection rate of calcification in patients with benign thyroid nodules was much lower than that in thyroid cancer patients.The difference between the two groups was statistically significant(P<0.05).The calcification type of the benign and malignant nodules of the selected 232 patients was compared with the calcification position,P<0.05,and the difference was statistically significant.The malignant nodules were compared with the fine calcification + central position,benign nodules and coarse calcification + marginal position,P <0.05,the difference was statistically significant.Through data analysis,it can be found that most of the malignant nodules are calcifiedin the central position,and the calcification type is generally fine-grained calcification;the benign nodules are mostly calcified at the edges,and the calcification type is mostly coarse-grained calcification.The above results were revalidated by data from another176 patients with thyroid calcification nodules collected during the same period.Clinical diagnosis using calcified particle size and calcification position indicators revealed that for thyroid malignant nodules,the sensitivity of fine-grain calcification and central location reached 81.3%(95% CI: 67.0,95.5),and its specificity was also high.89.3%(95% CI: 80.9,97.6);for benign thyroid nodules,the sensitivity and specificity of coarse grain calcification and marginal location were also high,the specific value was 80.4%(95% CI: 69.6,91.1)and 90.6%(95% CI: 79.9,101.3).The statistically significant factors in the univariate analysis of the 240 patients who were selected first and the 232 patients who were selected were substituted into the logistic regression analysis.The substitution factor was calcification type.Calcification position,gender,results showed that women(OR: 7.272,95% confidence interval: 1.94 ~ 9.581,P <0.05),fine calcification(OR: 7.156,95% confidence interval: 1.65 ~ 10.75,P <0.05)Central calcification(OR: 4.245,95% confidence interval: 1.44~9.141,P<0.05)were independent risk factors for malignant nodules.Conclusion: The fine particles + central position characteristics of calcification foci and the coarse particles + edge position characteristics of calcification foci can enhance the specificity of diagnosis on the basis of guaranteeing the sensitivity.The use of CT to detect calcification can save the cost of medical treatment for patients.Thyroid calcification can also be found in time when carrying out other CT examinations of chest,head and neck for patients,so as to improve the accuracy of diagnosis for patients.It has a very high value in clinical diagnosis of thyroid nodule calcification.
