Objective: In today's society,the phenomenon of population aging is becoming more and more serious,and the demand for restoration of complete dentures is increasing.The edentulous jaw patients continue to absorb the alveolar bone after the missing teeth,and the mandibular bone absorption is more obvious than that of the maxilla.Severe bone resorption will inevitably lead to the decline of the complete denture retention and stability,affecting the repair effect,and its production has become a repair collar.At present,textbooks,BPS and adsorptive denture theory systems often encounter nightmares of mandibular migratory pain when treating patients with III and IV edentulous jaws.This study attempts to continuously explore,practice and summarize the complete denture restoration of a large number of mandibular severe bone resorption patients with edentulous jaw.The theoretical system of severely difficult complete denture restoration,in order to reduce the difficulty of complete denture restoration in such edentulous patients,reduce the patient's pain and number of visits,and improve the quality of repair.Methods: From September 2017 to February 2019,17 patients with mandibular severe bone resorption edentulous jaws at our department were selected as subjects.By making personalized trays,silicone rubber was used to make mandibular pressure.Final impression,try to compress the soft tissue and get the approximate bone.The impression ensures that the denture tissue surface fits as close as possible to the uneven alveolar bone,reducing the interference of the bone tip.At the same time,the occlusal bite plate was used to record the relationship between the jaws,and the final complete denture was made.The number of pain points and the number of visits were observed.The observation period is 1 month.Through the feedback of the case,the operation process is continuously revised,explored and summarized.Results: The mandibular pressure impression is made by using silicone rubber,so that the denture tissue surface in the functional state fits as much as possible to the uneven alveolar bone surface.Innovative riding occlusal plate to avoid waxing and the extension of the mandible,while estimating the space of the teeth.The outer edge of the base extends 2-5mm from the outer oblique line to obtain more support area.The anterior teeth of the edentulous jaw should be 5-7 mm before the midpoint of the incisor mastoid to obtain a good anterior protrusion.Nine times of percussion,comprehensively grasp the scope and extent of the buffer,predict the retention,predict nausea sensitivity,check the suitability of the base edge,check the high point of the bite.The flexible use of the three occlusal planes ensures the aesthetics of the anterior region while avoiding too much or too little space in the posterior region.Verify the stability of the denture when trying the teeth and avoid the warping when chewing.Obtain a polished surface shape to avoid late food retention.Through the above main operation methods,the occurrence of tender points is effectively reduced or even avoided,and a complete denture with high stability is obtained.This led to the initial construction of a theoretical system of extremely difficult complete dentures.Conclusion: The complete denture made by the extremely severe denture theoretical system with the pressure impression as the core can effectively reduce the tenderness point,provide a stable and comfortable high-quality denture for the edentulous patients.The success criteria for edentulous complete denture restoration: can eat no pain,stability overwhelming.
