Discussion On The Clinical Effect Of Parurethral Drainage For Hypospadias Surgery

Objective To explore the clinical effect of new paraurethral strap drainage in accelerating postoperative recovery and improving the success rate of hypospadias surgery?Method In this article,80 cases of hypospadias admitted to our hospital from 2014-2018 year were analyzed retrospectively,Among them,40 cases of hypospadias were treated with Duckett procedure or Duckett procedure combined with Duplay procedure using the new paraurethral drainage,The other 40 cases underwent Duckett procedure or Duckett procedure combined with Duplay procedure,but new drainage was not performed,both groups had airbag catheter stent drainage in the new urethra.The new parafusal drainage method is to leave a leather strip next to the new urethra.The clinical data and curative effects of the two groups were compared to explore the clinical efficacy of the new method.Result There was no significant difference in the total number of cases allocated,the age of children,the proportion of hypospadias types and the development of glans penis between the trial group and the control group.The mean age of the experimental group was 2.8 ±0.55 years)and that of the control group was 2.9 ±0.45 years);31 cases(77.5%)of type II and 9 cases(22.5%)of type III in the experimental group;33 cases(82.5%)of type II and 7 cases(17.5%)of type III in the control group;32 cases(80%)of Duckett procedure in the experimental group,and 33 cases(82.5%)of Duckett procedure in the control group.The others are Duckett procedure combined with Duplay procedure.The above data were not statistically significant.There were significant differences between the experimental group and the control group in the time of removing the wrapping,the incidence of penile skin edema and the average time of removing the urethral stent(P < 0.05).The average removal time of the test group was 3.1±0.3 days,and that of the control group was 5.1±0.2 days.There was significant statistical significance between the two groups.No case of penile skin edema occurred in the test group,while 40 cases in the control group.The average indwelling time of urethral stent in test group was 11.1±0.6 days,while that in control group was 13.8±0.4 days.The two sets of data were statistically significant.The indexes of the experimental group were significantly better than those of the control group,which indicated that the new method would significantly accelerate the recovery process of the children after surgery.The absolute number of urinary fistula in the experimental group was also smaller than that in the control group,but the incidence of urinary fistula was not statistically significant because the sample size was not large enough.Conclusion In hypospadias operation,skin strip drainage beside the new urethra can solve the problem of postoperative wound exudation and urethral fistula,shorten the postoperative recovery time,accelerate wound healing,and should improve the success rate of the first stage operation.
