PURPOSE: To study the relationship between serum apolipoprotein A-?concentration and prognosis in patients with advanced hypopharyngeal carcinoma before the initial treatment,in order to find new prognostic factors,and to achieve a more comprehensive assessment of the prognosis of advanced hypopharyngeal cancer.MATERIALS AND METHODS: A retrospective analysis of 45 cases of advanced hypopharyngeal carcinoma patients who underwent neoadjuvant chemotherapy + surgery + radiotherapy from January 2010 to April 2017 at the First Affiliated Hospital of Fujian Medical University.Collect all medical records,including alcohol and smoking history,age,BMI,TNM staging of hypopharyngeal carcinoma,cervical lymph node metastasis,serum Apo A-? concentration,overall survival and recurrence.According to the median level of serum Apo A-?concentration,all patients were divided into a group(Apo A-?<1.24g/L)22 cases and b group(Apo A-??1.24g/L)23 cases.All data were statistically analyzed using SPSS 25.0 statistical software.The count data and the grade data test were compared using the Fisher's exact test and the rank sum test to compare the differences between the clinical indicators in the a and b groups.The Kaplan-Meier method was used to calculate and use the log-rank method for univariate analysis.The Cox proportional hazard regression model was used to study the effects of multiple factors on the survival and recurrence of advanced hypopharyngeal carcinoma.P<0.05 was considered statistically significant.RESULTS:1.Univariate analysis showed that overall survival rate and locoregional recurrence-free survival rate of advanced hypopharyngeal carcinoma was closely related to serum Apo A-?concentration,cervical lymph node metastasis and clinical TNM staging(P < 0.05),and not related to gender,age,BMI,smoking history,drinking history and pathological grade.2.Cox regression analysis showed that serum Apo A-?concentration was correlated with the prognosis of advanced hypopharyngeal cancer,and it was an independent factor affecting the overall survival and locoregional recurrence-free survival of advanced hypopharyngeal cancer.3.In a univariate and multivariate analysis,it showed that serum Apo A-?concentration and cervical lymph node metastasis were independent factors influencing recurrence of advanced hypopharyngeal cancer.CONCLUSION:.The serum Apo A-?concentration before initial treatment was an independent factor affecting the prognosis of advanced hypopharyngeal cancer patients.A decreased serum Apo A-?concentration indicates poor overall survival rate and locoregional recurrence-free survival rate,but high recurrence rate.
