Comparison Of Clinical Efficacy Of IL And DL Of Holmium Laser Ureteroscopic Lithotripsy In The Teatment Of Upper Ureteral Calculi

[Objective] This study is to investigate the clinical efficacy differences and the complications of the comparison of DL and IL of Ureteroscopic Holmium Laser lithotripsy in the treatment of Upper Ureteral Calculi.[Methods] To collect all patients of upper ureteral calculi from January 2016 to September 2018 in the first Affiliated Hospital of XIAMEN University.The same team of doctors treat the patiets by Ureteroscopic Holmium Laser lithotripsy.If the gravel posision is at ureteral segments,the group is In situ lithotripsy(IL).If all or part of the stones retrograde to the kidney and we must process it kidney by Flexible ureteroscopy at the kidney,the group is Displaced lithotripsy(DL).Collecting and organizing the clinical data and the medical information and postoperative complications,and analy the data between two group by the SPSS software.[Results] Included in this study is 151 cases,including 81 cases(M 55,F 26)in DL Group and 70 cases(M 46,F 24)in IL Group.The sex ratio is 2:1(M:F).IL(87.7%)has higher stone free rate than DL(97.1%),P?0.05,the diffirence between two group is statistically significant.The operation time: DL 59.51±18.72 min,IL 43.93±20.75 min,P?0.05,the diffirence between two group is statistically significant.The hospital costs: DL 33918.85±4450.57 yuan,IL 29983.66±8529.73 yuan,P?0.001,the diffirence between two group is also statistically significant.There is no statistically significant of hospital stay days between them.Postoperative complication,perinephritis: DL 20(24.7%)cases,IL 8(11.4%)cases,P?0.05,the diffirence between two group is statistically significant.All patients were followed up for more than 3 months,and ureteral stricture,IL 4(5.7%)cases,DL o case,P?0.05,the diffirence between two group is statistically significant.[Conclusion] Both DL and IL are effective measures of treatment of upper ureteral calculi.Although DL can increase the operation time,hospitalization expenses and perinephritis rate,reduce the rate of stone free,the IL could increase the rate of ureteral stricture.So the surgeons choose surgery measures as appropriate.
