Analysis Of Early Pain Related Factors After Primary Artificial Knee Arthroplasty With Unilateral Primary Knee OA

Objectives:The causes of early knee pain after the first unilateral primary knee arthroplasty(TKA)were analyzed and summarized to provide clinical and surgical advice and help,so as to improve patients' confidence in artificial knee arthroplasty and postoperative comprehensive satisfaction Methods:Qinghai university hospital were retrospectively analyzed joint section between December 2017 and May 2019,a total of 120 patients with an average age of 62 years(range of 50 to 77 years)of the patients,all patients had unilateral TKA surgery,VAS pain in all patients at all times during the study follow-up evaluation methods for the assessment and knee joint HSS,other data including history records,physical examination,inflammatory markers,radiological assessment.Other general information includes BMI,height,age,sex,etc.VAS scores and knee HSS scores of patients with knee pain were recorded before surgery and at day 1,3,7,30,3,6,and 12 months after total knee arthroplasty.SPSS 20.0 statistical software was used for data analysis,and P < 0.05 was considered statistically significant.Results:1.At 1,3,7 and 30 days after knee replacement,VAS score at 3 months was negatively correlated with HSS score at 3 months after knee replacement.2.Operation time and BMI were positively correlated with VAS at 1,3 and 7 days after joint replacement.3.The age of the patients was negatively correlated with VAS scores at 1,3 and 7 days after the replacement.4.VAS scores of the patients decreased dynamically at 1,3,7,30,3,6 and 12 months after knee surgery;HSS score showed an overall upward change.Specific causes of pain: of the 120 patients in the study,17 had primary post-knee replacement pain,an incidence of 14.1%.Pain after knee replacement: 1.Knee joint infection in 4 cases(3.3%);2.2.Lumbar spine lesions: 2 cases(1.6%);3.There were 3 cases(2.5%)of knee joint flexion and axial instability,and knee joint dystonia.4.There were 2 cases(1.6%)of vascular disease with poor vascular function and thrombosis.5.Abnormal movement track of patella in 1 case(0.8%);6.Cause of knee extensor device in 2 cases(1.6%);7.Hip lesions: 1 case(0.8%);8.Pain in 2 cases(1.6%).Conclusions:1.The main causes of early knee joint pain after primary unilateral knee OA primary artificial knee arthroplasty are joint infection,lumbar lesions,knee joint dystrophy,vascular and internal and external factors(dysfunction,thrombosis,etc.);2.The main causes of intra-articular factors were articular cavity infection and the main causes of intra-articular factors were lumbar diseases.2.Early pain after joint replacement affects functional recovery after knee replacement;3.Operation time and BMI influence the pain after knee replacement;It is not clear that there is a clear relationship between knee pain and gender after knee replacement.The postoperative pain of the patient's knee joint gradually decreased with time,and the joint function gradually improved.
