Objective The purpose of this study was to retrospectively analyze the cases diagnosed as PFO combined with migraine and treated with percutaneous PFO interventional occlusion in cardiac macrovascular surgery department of General Hospital of Ningxia Medical University,summarize the clinical experience,and explore the clinical efficacy and safety of percutaneous PFO interventional occlusion treatment in patients with PFO combined with migraine.Methods Data were collected from 42 cases diagnosed with PFO combined with migraine and treated with interventional occlusion at cardiac macrovascular surgery department of General Hospital of Ningxia Medical University from January 2018 to December 2018.All patients were successfully blocked.There were 17 males and 25 females,with an average age of 39.83±8.343 years.Any patients who affected the outcome of the treatment were excluded from the study.In addition to routine examinations,all patients were required to complete transthoracic echocardiography(TTE)and contrast transthoracic echocardiography(c-TTE).Patients with PFO and migraine underwent percutaneous PFO interventional occlusion.After successful sealing,bedside echocardiography was performed immediately to evaluate the occluder and shunt status.After 1,3,6 and 12 months,the electrocardiogram,chest radiograph and cardiac echocardiography were reviewed to observe and evaluate whether the patient had any operative complications.At the same time,patients were given the Headache Impact Test-6(HIT-6)score before operation and 1,3,6 and 12 months after operation to evaluate the headache degree.The total score ranged from 36 to 78,indicating the degree of headache impact on the patient's life.Data processing adopts SPSS20.0 statistical software.The measurement data was expressed as mean ± standard deviation(x ± s),and comparison between groups was performed using independent sample t test,and setting P <0.05 indicated that the difference was statistically significant.Results This study included a total of 42 cases of PFO combined with migraine,including 17 males and 25 females,with an average age of 39.83 ± 8.343 years.All 42 patients underwent interventional occlusion therapy and the operation was successful.All successful patients underwent TTE during and after operation,showing that the occluder was in good position and shape,and no obvious shunt was seen.The patient did not show any obvious discomfort when he was discharged from the hospital,and no complications such as severe arrhythmia,bleeding,and cardiac perforation occurred during the follow-up.The HIT-6 score at 12 months after surgery was 38.67 ± 3.918 points,which was much lower than the HIT-6 score before treatment of 61.14 ± 2.992 points,and the results were statistically significant(P <0.05).Conclusion The application of interventional occlusion in the treatment of PFO combined with migraine has a significant clinical effect,which can significantly improve the migraine symptoms and the quality of life of the patients.In addition,complications after interventional occlusion are rare,postoperative recovery is fast and safety is high.For patients with PFO combined with migraine,if there is no obvious contraindications,PFO interventional occlusion can be considered clinically.
