Objective To verify the effects of lidocaine mucilage, dimethicone powder and pronase as auxiliary drugs before gastroscopy on the clarity, examination time, adverse reactions and other effects of mucous membranes of various parts of upper digestive tract during the examination process. To explore the appropriate drug preparation time and plan before gastroscopy in order to better serve the clinical work.MethodsPatients who underwent gastroscopy in the Gastroscope Room of the Cardiovascular and Cerebrovascular Disease Hospital of Ningxia Medical General University from November 2018 to September 2019 were selected and divided into groups by random number table method.According to the different drug preparation schemes before examination, they were divided into three groups:Group A lidocaine mucilage, group B dimethicone powder+lidocaine mucilage, group C pronase+dimethicone power+lidocaine mucilage.According to the different drug preparation time before examination, group A and group B were randomly divided into three subgroups:Take the medicine 10min,20min and30min before the examination respectively, namely A1,A2,A3,B1,B2 and B3.Statistical analysis was carried out on the degree of clarity of mucosa under the endoscope, examination time, adverse reactions after taking medicine, causes of examination and detection of diseases in each group of cases during gastroscopy. Results 1.General situation:There were 350 cases involved in and completed this study, including 150 males and 200 females.there was no statistical difference in sex and age among the three groups.(X~2=6.510,P=0.369;F=1.406,P=0.211).Among them,41.7%of the subjects underwent gastroscopy due to abdominal pain and abdominal distension.Followed by acid reflux and heartburn accounted for 20.3%,Physical examination accounted for 10.6%,nausea and vomiting accounted for 3.4%,abdominal discomfort accounted for 6.6%.2.The mucosal clarity and examination time were observed at different medication times before examination:in this study, the total score of mucosal clarity in group A was 9.10±1.58 in group A1,9.04±1.98 in group A2 and 9.58±1.70 in group A3,respectively;The time of gastroscopy was 425.36±44.92s in group A1,408.36±53.31s in group A2 and405.58±49.66s in group A3.In group A,there was no significant difference in mucosal clarity and examination time after taking medicine at three different times before examination(P>0.05).The total score of mucosal clarity in group B was 7.46±1.63 in group B1,7.48±1.71 in group B2 and 7.06±1.33 in group B3;The time of gastroscopy was 344.46±101.78s in group B1,356.96±82.61s in group B2 and 344.86±79.19s in group B3,In group B,there was no significant difference in mucosal clarity and examination time after taking medicine at three different times before examination(P>0.05).3.Different drug preparation schemes were used to observe the mucosal clarity and examination time before examination:In this study, the total score of mucosal clarity in group A and group B was 9.24±1.77 and 7.33±1.57 respectively;the examination time was413.10±49.86s and 348.76±88.01s respectively, the mucosal clarity under gastroscopy in group B was significantly better than that in group A,and the examination time in group B was shorter than that in group A,and there was significant difference between the two groups(P<0.05).The total scores of mucosal clarity in groups A2,B2 and C were 9.04 1.98,7.481.71 and 6.40 1.05 respectively, the examination time was 408.36±53.31s,356.96±82.61s and 315.76±47.71s,respectively,compared with group A2 and group B2,group C has better mucosal clarity than group A2 and group B2,and the examination time is obviously shortened.the difference between the three groups is statistically significant(P<0.05).4.Among the 350 patients,54 had adverse reactions, including 30 in group A,20 in group B,and 4 in group C.Nausea reaction was the main reaction, including 48 cases of nausea reaction,4 cases of cough reaction,1 case of dizziness and 1 case of palpitation. Conclusion 1.The administration of lidocaine gel and/or dimethylsiloxane oil powder within 10-30min before gastroscopy has little effect on the clarity and examination time of the mucosa under the microscope, and can achieve the same foaming removal effect.2.Before gastroscope examination, dimethylsiloxane combined with lidocaine gelatinaceae can effectively remove gastric foam, improve the clarity degree of mucosa under gastroscope and shorten the examination time. The addition of streptomycin granules can further improve the clarity degree of mucosa and shorten the examination time.
