Systematic Review About Application Value Of Single-port Laparoscopic Ovarian Cyst Removal In ERAS

Objective:A systematic review of published studies to evaluate the value of single laparoscopy versus conventional laparoscopic cystectomy in enhanced recovery after surgery.Methods:Design retrieval method for computer retrieval of relevant databases at home and abroad: English database(PubMed,Embase,Cochrane Library etc.);Chinese database(WANFANG,CNKL,VIP and,etc.),supplemented by other retrieval(manual retrieval,etc.).Relevant literatures published before September 2019 were selected through literature screening and quality evaluation to extract data,and the Review Manager 5.3 software was used for analysis.Results:A total of 24 literatures(1022 cases in the LESS group and 1139 cases in the TLS group)were included for analysis using RevMan 5.3 software.Intraoperative evaluation index.Results showed as follows:1.Intraoperative evaluation inde: the operative time of TLS group was shorter than that of LESS group [WMD=5.3,95%CI :(2.29,8.31),Z=3.45,P=0.0006 < 0.05].Intraoperative blood loss between the two groups have no obvious differences [WMD = 1.45,95%CI:(4.08,1.18),Z=1.08,P = 0.28 > 0.05].2.Postoperative evaluation index: the total hospitalization time of the LESS group was shorter than that of the TLS group [WMD=-0.92,95%CI:(-1.35,-0.49),Z=4.2,P < 0.0001 < 0.05].The incidence of postoperative complications in the LESS group was lower than that in the TLS group [OR=0.29,95%CI:(0.17,0.49),Z=4.61,P < 0.00001 < 0.05].LESS postoperative anal exhaust time is TLS group faster [WMD=2.23,95% CI: 4.25,0.22),Z = 2.17,P = 0.03 < 0.05];Postoperative pain scale(visual analogue scale)of LESS group was lower than that of TLS group [WMD=-1.53,95%CI:(-1.96,-1.11),Z=7.10,P < 0.00001 < 0.05].Conclusion:The overall advantage of single-port laparoscopic surgery compared with traditional laparoscopy in ovarian cyst stripping is consistent with ERAS 'concept of intraoperative operation and postoperative recovery.
