| Objective:This study aims to design and develop a mobile application(APP)platform to promote the diet management of cancer patients through cooperation with a science and technology company,and applied it to the patients with head and neck cancer radiotherapy,evaluation based on the "Internet + diet management" model for improving the diet of patients with head and neck cancer radiotherapy,nutritional status,treatment effect,to participate in the activities of treatment in patients with cognitive level and quality of life.Methods:This study consists of two parts: mobile application development and clinical intervention.In the first part,through the methods of literature analysis,semi-structured interview and expert meeting,we cooperated with a technology company to complete the design and development of mobile APP platform.The second part was clinical intervention.Patients with head and neck malignant tumors who received radiotherapy in a grade a hospital of Shanxi Province from December 2018 to October 2019 were selected as the research subjects.According to admission order 36 cases were divided into control group and test group and control group in the implementation of regular diet management,the experimental group on the basis of conventional management,based on the mobile phone APP to diet management mode,the first week after admission,radiation treatment in the second week,radiotherapy one week after the end of the fourth week,radiotherapy for its energy intake,protein intake,PG-SGA score,albumin,prealbumin,total protein,BMI,and incidence of interruption of treatment,hospitalization days,to participate in the activities of treating cognitive level and quality of life for a month after discharge for comparative analysis.Data processing was mainly conducted by using Excel 2016 and SPSS 25.0 software for sorting and analysis.Counting data was expressed by frequency/percentage and measurement data by mean±standard deviation.The statistical methods mainly adopted t test,chi square test and repeated measurement hair difference analysis for statistical test(=0.05;P<0.05 was statistically significant).Results:1.Completed the design and development of mobile APP platform.From December 2017 to December 2018,I completed the design and development of the mobile APP platform,which is equipped with a medical terminal and a patient terminal.In addition to the mobile terminal,the medical terminal is equipped with a web terminal.Medical APP to view personal information,clinical diagnosis,complete nutritional screening and assessment,diagnosis,dietary nutrition prescription diet in patients with Settings,real-time monitoring and management,view after radiotherapy reaction and online communication functions,such as web client can access to more detailed information,and can all diet management and nutrition course summary for each patient records,etc.;The patient side consists of 5 modules: personal information,home page task(diet record,treatment process,etc.),community module display,message module and online follow-up.It can run normally by trying it out internally.2.The diet management mode based on mobile phone APP has been applied in patients with head and neck malignant tumor radiotherapy and achieved good results.Respectively in patients with head and neck malignant tumor radiotherapy andimplement regular diet management based on mobile phone APP diet management mode,the result shows: There was no significant difference in the general information of patients and the energy-protein intake in the first week of admission between the two groups(P >0.05),comparable;from the second week of radiotherapy,the energy-protein intake and nutritional indexes of the experimental group were better than those of the control group at each time period,and the results were statistically significant(P < 0.05);The results of repeated measures anova showed that the energy and protein intake of all patients changed significantly with the change of time,and the time effect was significant(P<0.05).The main effect of the intervention was significant(P< 0.05).There was an interaction between the time and the groups,which was statistically significant(P< 0.05).Compared with the control group,the differences in pg-sga score,albumin,pre-albumin,total protein and BMI in time,intergroup and interaction were statistically significant(P<0.05).In addition,the incidence of treatment interruption,length of stay,cognitive level of participating in treatment activities,and quality of life 1 month after discharge in the experimental group were all better than those in the control group,and the differences were statistically significant(P < 0.05).Conclusion:Based on the mobile phone APP to diet management for patients with head and neck malignant tumor radiation therapy,which can effectively improve its diet,increase energy-protein intake,is beneficial to maintain good nutritional status,reduce the happening of the interruption of treatment,and through the improvement of patients to participate in the activities of treatment the cognitive level,promote the smooth completion of the treatment,reduce hospitalization days,to improve the patients quality of life and satisfaction also is of great significance. |
