Objective:To investigate the relationship between pain and basic demographic information,partial cytokine levels,absolute Th17 cell counts,absolute Treg cell counts,Th17 / Treg ratio,anxiety,depression,and sleep disorders through the investigation of pain in RA patients Relationship,study the influencing factors of pain in RA patients,analyze the relationship between pain and non-inflammatory factors in RA patients,and provide new ideas for the treatment of RA.Methods:A total of 120 patients with RA were included in this study.Their venous blood samples were taken in the early morning on an empty stomach.Flow Cytometric Analysis was used to detect the absolute cell count of some subsets of peripheral blood CD4 + T cells.ELISA)to detect some cytokines in the serum of RA patients,and collect related laboratory indicators of patients at the same time,including red blood cell sedimentation rate(ESR),C-reactive protein(CRP),rheumatoid factor(RF),anti-cyclic citrullinated peptide antibody(Anti-CCP antibodies).Basic demographic information of RA patients,VAS score,Hospital Anxiety and Depression Scale(HADS),Pittsburgh Sleep Quality Index Scale(PSQI)and other contents are recorded in the form of questionnaires.Basic demographic information of patients includes name,gender,There are 8 aspects of age,marital status,occupation,education,personal monthly income,and course of illness.Results:1.Among the 120 patients with RA,mild,moderate,and severe pain accounted for5.8%(7/120),30.8%(37/120),and 63.3%(76/120)of all RA patients;in 58 patients with anxiety disorder,Among patients,mild,moderate,and severe pain accounted for 0%(0/58),17.2%(10/58),and 82.8%(48/58)of all RA patients with anxiety disorder,respectively;in49 patients with depressive RA Moderate,moderate,and severe pain patients accounted for2.%(1/49),12.2%(6/49),and 85.7%(42/49)of all RA patients with depressive disorder,respectively;among 48 RA patients with sleep disorders Patients with mild,moderate,and severe pain accounted for 2.1%(1/48),16.7%(8/48),and 81.3%(39/48)of all RA patients with sleep disorders,respectively.2.Comparison of basic demographic information: Pain in patients with RA is statistically significant in terms of age,personal monthly income,and duration of disease(P <0.05,P values are P = 0.04,P = 0.002,P = 0.001)3.Comparison of laboratory indicators: Pain in RA patients was statistically significant in terms of ESR,IL-2,IL-6,TNF-?,Th17,Th17 / Treg(P <0.05,P value was P= 0.001,P = 0.002,P <0.001,P <0.001,P <0.001,P = 0.002).4.Comparison of non-inflammatory factors: Pain in patients with RA was significantly different in terms of anxiety disorder,depression disorder,and sleep disorder(P <0.05,P values were P <0.001,P <0.001,P = 0.001).5.Correlation analysis: Pain in patients with RA was positively correlated with age,duration,ESR,IL-2,IL-6,TNF-?,Th17,Th17 / Treg levels and anxiety disorders,depression disorders,and sleep disorders(r = 0.225,0.302,0.278,0.323,0.361,0.447,0.394,0.305,0.404,0.382,0.305,P <0.05),and negatively correlated with income(r =-0.274,P <0.05).6.Logistic regression analysis: ESR(OR: 1.063,95% CI: 0.024 to 0.098,P = 0.001),TNF-?(OR: 1.038,95% CI: 0.002 to 0.072,P = 0.037),Th17 / Treg(OR: 39.291,95% CI:0.504 ~ 6.837,P = 0.023),anxiety disorder(OR: 0.157,95% CI:-3.228 ~-0.477,P =0.008),depression disorder(OR: 0.101,95% CI:-3.522)~-1.061,P <0.001),sleep disturbance(OR: 0.166,95% CI:-3.018 ~-0.578,P = 0.004)were independent risk factors for pain severity in RA patients.Conclusion:1.Pain is the main clinical manifestation of RA patients,and the incidence of severe pain is higher in RA patients.2.Independent risk factors for pain severity in RA patients were ESR,TNF-?,Th17 /Treg,anxiety disorder,depression disorder and sleep disorder.3.Among the factors that affect the pain of RA patients,non-inflammatory factors should not be ignored.Fully consider these influencing factors and put forward suggestions to reduce the pain of RA patients through psychological intervention and sleep improvement measures,so as to provide new opportunities for clinicians to carry out clinical diagnosis and treatment Ideas.
