Objective: To investigate the incidence of insomnia in psychological outpatients with anxiety disorders,explore the possible risk factors and protective factors for insomnia comorbidities in patients with anxiety disorder-For the generalized anxiety disorder,as the most common subtypes of anxiety disorders,we want to find the features and difference for sleep quality and sleep structure among medication patients group,non-medication patients group and healthy control group and provide theoretical support for the application of objective evaluation tool CPC in clinical sleep monitoring,early clinical screening and early intervention of anxiety disorder,further provide a theoretical basis for the clinical treatment of patients with anxiety disorders.Methods:Subjective assessment tools and objective assessment tools were used in combination.The subjects for subjective assessment tool study were 205 patients with anxiety disorder who met the enrollment requirement in the Mental Health Center of Shanghai Mental Health Center psychological clinic.Through using the general demographic questionnaire,clinical features data questionnaire,drug use questionnaire,clinical anxiety scale,sleep-related scale and other scales as the research tools,their situation in population characteristics,clinical symptoms,sleep status and other aspects were investigated.The subject for objective assessment tools were 60 patients with generalized anxiety disorder,who were recruited from 205 patients with anxiety disorders in the previous objective study,and also conformed to the CPC standard and volunteered to participate in the research.Finally,60 healthy subjects from the sample of CPC objective data collected from Taipei Veterans General Hospital in 2007 were selected to match with 60 patients with generalized anxiety disorder.Results:(1)There were significant differences in the onset age,comorbidity,current smoking and insomnia situation among the three subtypes of anxiety disorder.(2)And the aforementioned number were significantly correlated with the total score of HAMA(r = 0.873,P <0.01)and ISI(r = 0.467,P <0.01).(3)The older,generally in poor health,with poor energy,and has serious levels of anxiety,palpitation or rapid heartbeat symptoms.More adverse sleep awareness and attitude and introversion are independent risk factors for generalized anxiety disorder patients with insomnia.Seeking Chinese medicine adjuvant therapy,feeling greater interpersonal support and maintaining stable emotion and low degree of disguise are protection factors for generalized anxiety disorder patients with insomnia.(4)Compared with the health control group,the rate of stable sleep of the anxiety group was significantly lower(P <0.01),and the time of early falling asleep was also significantly longer(P <0.01).There was no significant difference in HFC,LFC and VLFC between the non-medication group and the healthy control group(P <0.01),but the time of early falling asleep was over 30 minutes and significantly longer(P <0.01).Conclusion:(1)As an objective assessment tool for sleep quality monitoring,CPC is feasible in clinical practice.(2)Insomnia affects patients with anxiety disorders commonly,especially for the patients with generalized anxiety disorder.(3)The higher the anxiety level of patients,the more institutions and departments will be involved.Patients with anxiety disorders usually seek medical treatment from the general hospital to the psychiatric hospital,and along with the Chinese traditional medicine during the therapy.The General hospitals should improve the identification of anxiety disorders,especially the panic disorder.(4)Compared to health control,the shallow sleep in patients with anxiety disorder who took the medicine increased,while the time for early falling asleep in patients who did not take medicine prolonged.
