Objectives:To investigate the correlation between remnant cholesterol(RC)level and Gensini score in patients with coronary heart disease(CHD)and its clinical significance,and to provide basis for its prevention and treatment.Methods:According to the results of coronary angiography,250 cases of suspected CHD patients in the Department of Cardiology,affiliated Hospital of North Sichuan Medical College from January 2019 to September 2019 were divided into normal coronary group(n=58),acute coronary syndrome(ACS group,n=95)and stable angina pectoris(SAP group,n=97)according to the results of coronary angiography,respectively.65 healthy people in the same period were selected as the healthy control group.The blood lipid and hypersensitive C-reactive protein levels were detected,remnant cholesterol was calculated as total cholesterol minus high-density lipoprotein cholesterol minus low-density lipoprotein cholesterol.The clinical data of the patients were collected,and the severity of coronary artery lesions was evaluated according to Gensini score.The levels of RC,blood lipid index and hypersensitive C-reactive protein in all groups were analyzed,and the correlation between RC,blood lipid index,hypersensitive C-reactive protein levels and Gensini score was discussed.According to RC quartile,the CHD group patients were divided into high RC group and low RC group.Furthermore,the subgroup relation analysis between RC level and Gensini score in CHD patients with low-density lipoprotein cholesterol less than 1.8 mmol/L were conducted.Results:1.The level of RC in patients with coronary heart disease was significantly higher than control group(P<0.01).The levels of RC in normal coronary group,SAP group,ACS group radually increased in turn,and the difference of which was statistically significant(P<0.05).2.The Gensini score of ACS group was higher than that of SAP group(P<0.01).3.The levels of RC,TG,TC,LDL-C,non-HDL-C,LP(a)were positively correlated with Gensini score(r=0.64,0.41,0.48,0.42,0.21,P<0.01);RC was a risk factor of severity of coronary artery lesions.4.No matter whether the level of LDL-C in patients with CHD was less than 1.8 mmol/L,the score of Gensini in high RC group was higher than that in low RC group(P<0.01).5.The levels of TG,TC,LDL-C,non-HDL-C and hs-CRP increased with the increase of RC level,while HDL-C level decreased.Conclusion:1.The elevated RC level is closely related to the occurrence and severity of CHD and may increases the risk of chronic coronary artery disease developing into acute coronary syndrome.2.RC level is positively correlated with Gensini score,which is a risk factor for the severity of coronary artery lesions.RC may be a potential indicator to judge the severity of coronary artery disease.3.RC level is closely associated with Gensini score in CHD patients under controlled LDL-C levels,being available as one of the concerned indicators of dyslipidemia and a possible indicator for monitoring lipid-lowering therapy in patients with CHD.
