Study On Estradiol And Abnormal Immune Function In Patients With Primary Biliary Cirrhosis

Objective:To explore the immunological abnormality of patients with Primary biliary cirrhosis(PBC),and analyze the correlation between estradiol and abnormal immune function,so as to provide clinical basis for seeking immunoadjuvant therapy of PBC.Methods:The estradiol and immunological data of 67 PBC patients and 80 healthy people in the Second Hospital of Shanxi Medical University were collected.The two groups were classified by age to analyze the abnormal estradiol and immune function and the correlation between estradiol level and abnormal immune function in the PBC group at various ages.Results:1.There was no significant difference in estradiol between PBC group and control group(P > 0.05).The difference of estradiol between different age groups in PBC group was statistically significant(P < 0.05),the highest estradiol level was in the age group ≤ 49 years old,and there was a negative correlation between estradiol and age(r =-0.494,P < 0.001).2.In the peripheral blood T lymphocyte subsets,the levels of Th/Ts and total B cells in the PBC group were higher than those in the control group,while the levels of total T cells,Ts cells and NK cells were lower than those in the control group,with statistically significant differences(P<0.05).3.In the CD4+T cell subgroup,the levels of Th1 cells and Th1/Th2 in the PBC group were higher than those in the control group,and Th2 cells were lower than those in the control group,with statistically significant differences(P<0.05).the levels of Th17 cells、Treg cells and Th17/Treg were not significantly different from those of the control group(P>0.05).4.In terms of cytokines,IFN-γ,TNF-α,IL-2,IL-4,IL-6,IL-10,IL-17 in the PBC group were all higher than those in the control group,with statistically significant differences(P<0.05).5.The Spearman rank correlation analysis showed that In the PBC group,estradiol was negatively correlated with the proinflammatory effects of B,Th1,Th17,IL-2,IFN-γ and TNF-α,and positively correlated with Th2,Th.It was negatively correlated with the anti-inflammatory effects of IL-10,and the difference was statistically significant(P<0.05).There was no significant correlation with T,Ts,NK,IL-4,IL-6,IL-17 and Treg(P > 0.05).Conclusion:1.The immune abnormality of T lymphatic system in PBC patients may be mainly manifested by the increase of Th1 cells and their main secreted IFN-γ,TNF-α,and reduction of Th2 cells.2.Estradiol is an important endogenous immune modulator.High concentrations of estradiol may correct immune imbalance by inhibiting Th1 cells and their inflammatory cytokine levels and promoting Th2 cells and their anti-inflammatory cytokine levels in patients with PBC.
