Background and objective:Postpartum hemorrhage is currently one of the leading causes of maternal death worldwide,and most deaths of bleeding can be prevented by related emergency techniques.Timely blood transfusion is an effective measure to reduce maternal mortality.However,the source of allogeneic blood in the world is tense,and allogeneic blood input has more adverse reactions.There have been many cases of autologous blood transfusion in patients with bleeding during cesarean section,and there have been no reports of serious related complications in clinical studies and retrospective analysis.However,maternal hemorrhage is often mixed with amniotic fluid,and the blood composition is complex.What to worry about the use of autologous blood transfusion are amniotic fluid embolism and immune responses caused by fetal red blood cells.The purpose of this experiment is to explore the effect of cell caver combined with leukocyte filter on the removal of amniotic fluid constituents and other harmful components in recovered blood,and to provide data and information for the promotion of autologous blood transfusion in patients with heavy bleeding during cesarean section.Materials and Method:Collect 42 blood samples of parturient women undergoing cesarean section from June 1,2019 to July 30,2019 in our hospital,use cell saver to recover the bleeding,after anticoagulation with heparin,divided the samples into four groups :control group,Filter group,washing group and filter+ washing group.Control group: Each blood sample is directly taken before washing.Filter group: No washing,after filtering through the leukocyte filter,the specimen is taken.Washing group: Wash the blood with double the amount of washing liquid to form a blood sample.Filter +washing group: the washed blood is filtered through a leukocyte filter and then put into a container.The hematocrit(HCT),alpha-fetoprotein,squamous epithelial cells,fetal fat,vellus hair,meconium and other amniotic fluid constituents and fetal components in the four groups of recovered blood samples were detected.Result:Using SPSS 17.0 for statistical analysis,P <0.05 means the difference has statistical significance.(1)Alpha-fetoproteinCompared with the control group,there was no statistically significant difference in the alpha-fetoprotein content of the filtered samples(P> 0.05).The alpha-fetoprotein content of the washing group and the filter + washing group decreased(P <0.05).Compared with the filter group,the content of alpha-fetoprotein in the washed samples decreased(P <0.05).There was no significant difference in the content of alpha-fetoprotein between the washing group and the filter + washing group(P> 0.05).(2)HCTCompared with the control group,there was no statistically significant difference of HCT after filtering and washing(P> 0.05).Compared with the filter group,the HCT of the samples increased after washing(P <0.05),and there was no statistically significant difference between the group after washing and filtering(P> 0.05).Compared with the washing group,there was no statistically significant difference of HCT after washing and filtering(P> 0.05).(3)Squamous epithelial cellsCompared with the control group,the squamous epithelial cells of the samples were reduced after filtering and washing(P <0.05).Compared with the filter group,the samples had more squamous epithelial cells after washing(P <0.05),and there was no statistically significant difference in the number of squamous epithelial cells between the washing and filter groups(P> 0.05).Compared with the washing group,there was a statistically significant difference in the number of squamous epithelial cells in the filter+ washing group(P <0.05).(4)Vellus hair,fetal fat and meconiumAt high magnification,vellus hair,fetal fat and meconium were seen in the control group,and vellus hair and fetal fat were seen in the washing group.In the filter group and the filter + washing group,fetal fat components were found,but no hair and meconium components were seen.In summary,it can be concluded that:1.The double washing solution of the blood recovery system can not completely remove the amniotic fluid components such as vellus hair and fetal fat,partially remove the alpha-fetoprotein,and cannot completely remove the inflammatory cells and squamous epithelial cells;2.Leukocyte-removing filter can completely remove vellus hair and other organic components,inflammatory cells and squamous epithelial cells,but cannot remove alpha-fetoprotein and fetal fat;3.The blood recovery system combined with the white blood cell filter can completely remove inflammatory cells,squamous epithelial cells and fetal structure,and partially remove alpha-fetoprotein and fetal fat.
