Purpose To explore the clinical features,diagnosis,treatment and the outcome of amniotic fluid embolism(AFE).Methods We retrospectivly studied the clinical manifestation,diagnosis,treatment and the outcome of 1 case of AFE in Peking University Shenzhen Hospital and previous 70 literature by introducing "amniotic fluid embolism" as keywords in the Pub Med search engines,The time frame of the search was limited from 1990 to 2019.Results The common risk factors of AFE patients were multipara(37 cases,52.9%),≥ 35 years(28 cases,40.0%),induced labor(15 cases,21.4%),etc.The main manifestations were shock(44 cases,62.9%),DIC(47 cases,67.1%),cardiac arrest in39 cases(55.7%),ARDS(26 cases,37.1%).Of the 70 patients with AFE,16 died and54 survived.Among the 70 patients with AFE,the treatment included mechanical ventilation(52 cases,74.3%),the implementation of vasoactive drugs(50 cases,71.4%),blood transfusion(43 cases,61.4%)and CRP(39 cases,55.7%).There were 11 cases(15.7%)of AFE patients treated with implementation of ECMO.We report a case of AFE followed by cardiac arrest at 39 weeks of gestation.Conclusions AFE is an exclusive clinical diagnosis,The main manifestations of AFE are circulatory failure,respiratory failure and coagulation failure.Early recognition,high-quality cardiopulmonary resuscitation and coagulation dysfunction treatment are the main treatment methods.
