Analysis Of Clinical Pathological Factors Of Intrahepatic Recurrence After Curative Resection Of Small And Micro Hepatocellular Carcinoma

【Purpose】 : To observe the type of recurrence of small and micro hepatocellular carcinoma after radical resection(≤5cm),to explore the clinical and pathological factors related to postoperative intrahepatic recurrence,and to provide reference for guiding postoperative anti-relapse therapy【Methods】: Retrospective analysis of 109 patients who underwent radical resection of hepatocellular carcinoma(HCC)with a single lesiona and a diameter of(≤5cm)from January 2001 to June 2013 in the First Affiliated Hospital of Fujian Medical University.Recurrence and metastasis and type of intrahepatic recurrence.By collecting medical records and follow-up data,based on statistical description,choose age,gender,peripheral blood AFP,NLR,PLR,ALB/PT,postoperative total bilirubin(TBIL)peak,MVI,Intraoperative hemorrhage,perioperative blood transfusion,location of tumor recurrence and number of recurrences,and many other clinicopathological factors that may affect prognosis,to perform χ2 test,single factor survival Kaplan-Meier analysis and multi-factor Cox proportional hazard model analysis,and Trying to find predictors of recurrence and metastasis of small and micro hepatocellular carcinoma.【Results】: 1.The recurrence rates at 1,3,and 5 years after radical resection of small and micro hepatocellular carcinoma were 24.77%,43.11%,and 46.78%,respectively.2.Statistical analysis of the type of recurrence after radical resection of small and micro hepatocellular carcinoma showed: 12 months after surgery as the time node analysis,recurrence group within 12 months and recurrence group after 12 months in recurrence type-total liver diffuse recurrence There was a significant difference in the number of cases with ≥2 recurrences(P<0.05).After 12 months,the number of cases with diffuse recurrence and recurrence of ≥2 recurrences in the recurrent group decreased.3.The risk factors for intrahepatic recurrence,metastasis and survival after radical resection of small and micro hepatocellular carcinoma were recurrence type-diffuse recurrence,AFP>1210 ng/ml,NLR>7,PLR>96,ALB/PT<3,and the independent risk factor is ALB/PT<3.【Conclusions】: 1.Early recurrence of small and micro hepatocellular carcinoma after radical resection(within 12 months)with total liver diffuse recurrence and recurrence of lesions ≥ 2;2.ALB/PT may provide prognostic evaluation for radical resection of small and micro hepatocellular carcinoma.
