Objective: Explored the value of the T1 mapping imaging for the quantitative evaluation of liver lesions.To predict the tissue arrangement of small hepatocellular carcinoma based on the pathological level by T1 mapping imaging.Measured the signal intensity of lesions on before contrast-enhanced and hepatobiliary phase images,and analyzed it's relationship with pathological tissue classification of small hepatocellular carcinoma,to explore feasibility of the measurement method.Methods: Retrospectively collected images and clinical findings of 32 patients who conformed small hepatocellular carcinoma(? 3cm)by pathology and used Siemens Skyra 3.0T magnetic resonance imaging on the upper section of abdomen between February 2016 to February 2019 in the First Hospital of Lanzhou University.32 cases of single pathological tissue classification were included in the study.According to WHO hepatocellular carcinoma pathological tissue Type,the pathological tissue classification of SHCC was divided into three groups,including pseudo gland type,dense type and beam type.All cases of small hepatocellular carcinoma do common upper abdomen MRI scan,dynamic contrast enhancement scan and T1 mapping imaging before and after contrast agent injection.The T1 mapping image was processed by Siemens post-processing workstation,to measure the T1 value of the lesion.the liver-specific dynamic enhanced images by Gd-EOB-DTPA was transferred to the PACS system to measure signal intensity(SI).The T1 value and signal intensity before liver specific enhancement and after injection(delay 20 min)of the lesion were expressed by T1 pre,T1-HBP,SIpre and SI-HBP(Hepatobiliary phase,HBP),?T1% and ?SI% was calculated,?T1% =(T1pre-T1-HBP)/ T1 pre ×100%,?SI% =(SI-HBP-SIpre)/ SIpre ×100%.Statistical analysis,the collected data is tested by homogeneity of variance,and the statistical test method is selected according to whether the variance is homogeneous or not.The one-way analysis of variance(One-Way ANOVA)and non-parametric Kruskal-Wallis test were used to evaluate the variance of ? SI% and SHCC Pathological tissue classification,?T1% and SHCC Pathological tissue classification.The relationship between ?SI%??T1% and SHCC pathological tissue type was evaluated by Spearman.Result:1.There were 32 lesions in 32 patients,including 16 cases of pseudo gland type,6 cases of dense type and 10 cases of beam type.2.According to the pseudo gland type,dense type and beam type,The average value of ? SI% increased gradually,and the average value of ? T1% gradually decreased,by The average values images of ?SI% and ?T1% plotted by Spass23.0.3.Between the three groups of pseudo gland type,dense type and beam type,there was significant differences in ? T1%(H=11.032,P=0.004<0.05)and no significant difference in ?SI%(F=0.584,P=0.564>0.05).4.? T1% was moderate negative correlation with pathological tissue classification of small hepatocellular carcinoma(r=-0.526,P=0.002<0.01).There was no significant correlation between ? SI% and pathological tissue classification of small hepatocellular carcinoma(r=0.048,P=0.796> 0.01).Conclusion:1.By T1 mapping imaging to evaluate small hepatocellular carcinoma has certain clinical value.2.The ? T1% value obtained by T1 mapping imaging before and after the enhancement was moderately negatively correlated with the pathological tissue classification of small hepatocelluar carcinoma.It was preliminarily confirmed that T1 mapping imaging before and after the enhancement could predict the tissue arrangement of small hepatocelluar carcinoma.3.The ? SI% value obtained by before contrast-enhanced and hepatobiliary phase images was no significant correlation between the pathological tissue classification of small hepatocellular carcinoma.It was confirmed that the measurement method had little value on evaluating small hepatocellular carcinoma.
