Objectives1.To understand the current situation of family maternal and infant care providers(MICPs)during puerperal period and the experiences of postpartum women after receiving maternal and infant care services from MICPs.2.To analyze the occupational status,training needs and its influencing factors of MICPs.Methods1.Quantitative research combined with qualitative research were adapted.Quantitative method was used to explore the service satisfaction by self-designed 'Questionnaire on current situation of maternal and infant care services in Fuzhou city'.429 postpartum women who employed the family MICP during doing the month were recruited from postpartum clinic of six tertiary level hospitals in Fujian province to identify their satisfaction after receiving service from MICPs.2.Descriptive qualitative method was adapted to explored the women who receiving service experiences of being provided by family MICPs through the face-toface semi-structured interview.23 postpartum women were recruited and content analysis was used to sort out and analyze the interview data in order to further understand their service feelings and experiences.3.The 'Occupational status and training needs of family MICPs in Fuzhou city questionnaire' was initially developed based on the literature review and Delphi panel method.A convenience of sample of 130 family MICPs were recruited from nine domestic companies.t test,One-way ANOVA,Spearman correlation analysis and chisquare test were used in data analysis to figure out the occupational status,training needs and influencing factors of family MICPs.4.Descriptive qualitative method was adapted to further explored the occupational status and training needs of family MICPs through the face-to-face semi-structured interview.7 MICPs and 3 managers of domestic companies were recruited in our study and content analysis was used to analyze the interview data.Results1.Results show that service satisfaction of users with family MICPs was in a middle or even lower level.Merely 64.1% of mothers thought that MICP could meet their needs for maternal and infant care services at most of time.The top three items with the lowest satisfaction score items were respectively 'postpartum functional exercise guidance,such as pelvic floor functional exercise'(65.9%),'postpartum physical recovery guidance'(66.3%),and 'neonatal sensory perception development'(72.6%)respectively.Five themes were identified from the qualitative interviews which includes: there was a gap between the service provided by family MICPs and service expectation of postpartum women,the quality of service provided by family MICPs varies,MICPs' interpersonal communication skills needs to be improved,family MICPs played an important role in the maternal family,and positive experience of service attitude.2.Self-designed 'Occupational status and training needs of family MICPs in Fuzhou city questionnaire' which includes maternal and child care professional basic knowledge,maternal care professional knowledge and skills,neonatal care professional knowledge and skills,and professionalism.Four dimensions consist of 30 items,Cr = 0.89,S-CVI was 1.00,I-CVI ranged from 0.84 to 1.00,which demonstrated its good content validity.The Cronbach '? value of each dimension was all more than 0.70,and the retest reliability was 0.87.3.Result indicates that the average length of family MICPs(6.18±3.64 years),and the number of family MICPs whose education level under junior high school accounted for 89.3%.In terms of employment status,52.3% received orders privately,48.5% relied on intermediaries,and 27.7% were employees of domestic companies.The average monthly income of individuals was over 7,500-yuan accounting for 62.3%.78.5% of subjects had high professional recognition.The highest dimension of training needs is Professionalism dimension(15.55±3.77).The scores of the top three items for training needs were: 'relevant legal knowledge','dietary making',and 'prevention and care of common neonatal diseases'.There was a negative correlation between training needs and MICP' s ages as well as working years(P<0.05).In addition,there were statistically significant differences in different education levels and levels of MICPs about the necessity of regular training.(P < 0.05).Three themes based on the framework of ERG theory were identified from the result of qualitative interviews concerning the feelings of training needs of family MICPs which includes: physiological needs,interpersonal communication needs,and occupational development needs.Conclusions1.The general satisfaction of maternal and infant care services still need to be improved,especially in items of the postpartum pelvic floor muscle rehabilitation guidance,guidance of breastfeeding and the development of neonatal perception development as well as the interpersonal communicational skills should be highlighted.2.The general quality level of family MICPs was low,working process lacked standardized management and training,and the willingness of group training was not strong.It is suggested that the government departments should enhance market supervision and standardize market order.According to the level of maternal and infant care workers,the professional continuing training should be strengthened of MICPs in the future.
