Purpose: To improve the accuracy of histological classification of renal cell carcinoma(RCC)by comparing the pathological types of RCC with the imaging data of MRI,so as to provide an accurate basis for clinical diagnosis and treatment.Material and Method1.Objective: The retrospective analysis was made of 100 cases of space-occupying renal lesions(72 males and 28 females,with an average age of 55 ± 12)who were treatedtreated in The 900 th hospital of Joint Logistic Support Force,PLA Region from January 2015 to November 2018.All patients were operated on and confirmed by pathology and they were scanned by routine MR,DCE-MRI and DWI before operation.2.Research method: The German Siemens Magnetom Skyra 3.0T superconducting MR scanner with 18-channel body coil was used.All patients were scanned by routine MR in turn: VIBE sequence on T1 WI,Blade sequence on T2 WI,T2WI HASTE sequence on coronal position and DWI sequence(fat-suppressed SS-EPI).VIBE sequence was used for dynamic enhancement scanning.A retrospective analysis was made of 100 cases of renal cell carcinoma confirmed by operation and pathology.The pathological manifestations of the tumors were comprehensively analyzed with the measurement of DWI images and the recording of ADC values of the lesions on plain,enhanced and scans,and the differences were tested with statistical software.Results: Pseudocapsule could be found in papillary renal cell carcinoma,clear cell carcinoma and chromophobe renal cell carcinoma,and there was no significant difference between groups(P > O.05).In addition,there are statistical significance in cystic degeneration,hemorrhage and necrosis between chromophobe renal cell carcinoma and clear renal cell carcinoma,papillary renal cell carcinoma and chromophobe renal cell carcinoma(P < 0.05).There are no statistical significance in cystic degeneration,hemorrhage and necrosis between papillary renal cell carcinoma and clear renal cell carcinoma(P > O.05).MR plain scan signal characteristics T1WI: clear renal cell carcinoma was more common with mixed signals,chromophobe renal cell carcinoma was more common with equal low signals,and there was statistical significance between them(P < 0.05);T2WI: there is statistical significance in chromophobe renal cell carcinoma?clear renal cell carcinoma ?papillary renal cell carcinoma(P < 0.05).The enhancement modes of enhanced scanning: papillary renal cell carcinoma showed slight enhancement,transparent cell carcinoma mainly showed moderate and severe enhancement,and chromophobe cell carcinoma showed a higher proportion of mild and moderate enhancement and moderately enhanced with statistical significance between and within groups(P < 0.05);There is statistical significance in signal intensity of papillary renal cell carcinoma,clear cell carcinoma and chromophobe cell carcinoma in cortical,medullary and excretory stages(P < 0.05).ADC values: There is statistical significance in ADC values among papillary renal cell carcinoma,clear cell carcinoma,and chromophobe cell carcinoma(P < 0.05).Conclusion: The clinical and pathological findings of different subtypes of renal cell carcinoma are basically consistent.The analysis of cystic degeneration,necrosis and hemorrhage,as well as the characteristic manifestations of dynamic contrast-enhanced magnetic resonance imaging are of great value in the diagnosis and differential diagnosis of renal cell carcinoma subtypes.the ADC value is helpful in differentiating renal cell carcinoma subtypes.
