?Objective?By analyzing the postoperative pathological features of papillary thyroid carcinoma with and without micro-calcification,to explore whether PTC with micro-calcification is more invasive and provides a reference for clinical treatment options.?Method?190 patients with papillary thyroid carcinoma who had undergone primary surgery and been confirmed by postoperative pathology from January 2018 to December 2018 in Fujian Provincial Hospital were enrolled in our study.We retrospectively analyzed the basic clinical information,ultrasound reports,postoperative pathological reports,BRAF V600 E genes,immunohistochemistry reports and prognoses.Statistical analysis was performed on the collected data using SPSS 24.0 statistical software package.Single factor analysis was performed by chi-square test and multivariate analysis was performed by binary logistic regression analysis.The differences of postoperative pathologic features and prognosis were compared between PTC with and without micro-calcification in terms of gender,age,tumor size,multifocal tumor,tumor boundary,capsule invasion,local tissue invasion,central lymph node metastasis,tissue with chronic lymphocytic thyroiditis or nodular goiter,BRAF gene mutation,CK19,Galectin-3,MC,risk grading and prognosis assessment(AMES,MACIS).?Result?1.A total of 190 patients were included in the study.Among them,121 cases were PTC with micro-calcification,and 69 cases were not.The proportion of age <45 was higher in the group of PTC with micro-calcification,and the difference was statistically significant(=4.192 P<0.05).There was no significant difference between the two groups in the analysis of 55-year-old group and gender(P>0.05).2.In the micro-calcification group,the rate of CLNM,the ratio of tumor diameter >6 mm,and the proportion of combined CLT were higher than those in the non-micro-calcification group,and the difference was statistically significant(P< 0.05).There was no significant difference in aspect of multifocal tumor,capsule invasion,tumor boundary,local tissue invasion,tissue with nodular goiter,BRAF gene mutation,CK19,Galectin-3,MC,risk grading,prognosis evaluation(MACIS and AMES)(P>0.05).3.Binary Logistic multivariate analysis showed that the rate of central lymph node metastasis and tumor diameter >6 mm in the micro-calcification group were significantly higher than those in the non-micro-calcification group,the difference was statistically significant(P<0.05).There was no significant difference between the two groups in terms of age and tissue with CLT(P>0.05).?Conclusion?PTC with micro-calcification has poor clinic-pathological features,and is more invasive than PTC without micro-calcification.It has a higher rate of central lymph node metastasis and a larger tumor diameter.Therefore,for patients of PTC with micro-calcification,the thoroughness of central lymph node dissection should be paid attention to during surgery.
