Objective: To explore the relationship between EGFR gene mutation and clinical features in in lung adenocarcinoma.Methods: A retrospective cross-sectional study was conducted on 417 cases of lung adenocarcinoma confirmed by paraffin-embedded clinical pathology in thoracic surgery from January 2015 to August 2018.There were 209 EGFR 19 and 21 mutations and 208 wild type mutations.194 males and 223 females.All the selected subjects were given general information including medical history,smoking history,fasting venous blood in the morning,CEA and other indicators.EGFR gene was detected in tumour pathology,and the size,location and pathological type of tumors were recorded.The differences of sex,age,tumor location,size,pathological subtypes and smoking history between groups were compared.The influencing factors of effective mutation of EGFR gene were analyzed by binary logistic regression.Results: 1.Comparing the clinical characteristics of EGFR effective mutation group and EGFR gene non-mutation group,binary logistic regression analysis showed that gender and pathological subtypes(infiltration/microinvasion)were the influencing factors of EGFR gene mutation in lung cancer.There was significant difference in age between 19 and 21 mutations,but no significant difference in other factors.2.By binary logistic regression analysis of all subjects according to gender subgroups,smoking index was found to be the influencing factor of effective mutation of EGFR gene in male patients,and pathological subtypes(infiltration/microinvasion)were the influencing factors of effective mutation of EGFR gene in female patients.3.Bivariate logistic regression analysis showed that smoking index was theinfluencing factor for the effective mutation of EGFR gene in smokers,while gender and pathological subtypes(infiltration/microinvasion)were the influencing factors for the effective mutation of EGFR gene in non-smokers.4.In smokers with 1000?SI,the effective mutation rate of smoking index and EGFR gene of lung cancer was significantly lower than that in smokers with SI<600 ?600?SI<1000.Conclusion: 1.For lung adenocarcinoma patients,gender,smoking status and pathological subtypes are the influencing factors of EGFR.2.Smoking index is significantly correlated with EGFR mutation status in smokers.With the increase of smoking index,the possibility of EGFR mutation is less.Gender and pathological subtypes are the influencing factors of EGFR mutation status in non-smokers.The mutation rate of EGFR in smokers with 1000 < SI decreased significantly.
