Objective To explore the curative effect of balloon sinuplasty in the treatment of headache with sinus structural abnormality,and to analyze the effects of balloon sinuplasty on the symptoms of headache,quality of life and local nasal mucosal recovery,so as to provide reference for the treatment selection of patients with sinus structural abnormality headache.Methods The clinical data of 25 patients who were admitted to our department from March 2018 to February 2019 and diagnosed with headache due to abnormal sinus structure were retrospectively analyzed.All patients underwent balloon sinuplasty surgery under general anesthesia,and all surgeries were performed by the same senior doctor.According to the intraoperative details,whether to perform functional endoscopic sinus opening surgery at the same time or not was determined.Except for the patients who underwent balloon sinuplasty alone,all the other patients received conventional postoperative hemostasis by nasal packing,which was removed 1-3 days later,intravenous antibiotics 1-2 days later,and nasal irrigation twice a day after the removal of nasal packing.Outpatient review and telephone contact were used,and postoperative follow-up was 1~11 months.Surgical duration,operative blood loss,postoperative discharge time,postoperative complications and other indicators were retrospectively collected to comprehensively analyze the surgical effect.Electronic endoscopic or sinus CT examinations were reviewed regularly(1,3 months)after surgery,and the efficacy of balloon sinuplasty in the treatment of sinus structural headache was analyzed using the 0-10 visual analogism scale(VAS),the sino-nasal outcome test-20(Snot-20),and the nasal endoscopic Lund-Kennedy scale.Wilcoxon symbolic rank sum test in SPSS 20.0 software was used for statistical analysis of the data.Results Of 25 patients,16 were males and 9 were females,with an average age of 36.2(±12.3 SD)years.The mean course was 4.5 years.Median follow-up was 5 months(rang 1-11).After sinus balloon surgery,the patients' headache VAS score,snot-20 score,snot-20 headache score and the nasal endoscopic Lund-Kennedy score were improved from baseline(P<0.05).Compared with the treatment group of sinus balloon dilation and the treatment group of sinus balloon dilation combined with functional endoscopic sinus surgery,there was no significant difference in the efficacy between the two groups,with significant improvement in subjective symptoms and headache.Postoperative nasal endoscopy showed that the opening of the sinus was good,no mucosal edema,no polyp formation,no nasal secretion accumulation,no scab skin formation,good morphology of the sinus,and no scar adhesion.Postoperative sinus CT showed no thickening of sinus mucosa,unobstructed sinus ostia,and no secretion accumulation or neoplasm in sinus cavity.No serious adverse reactions and complications occurred in all patients,and no patient required further surgical treatment.Conclusion Balloon sinuplasty in the treatment of sinus structure abnormal headache,with good mucosal protection,small trauma,less bleeding,the short length of the operation,and so on,for the treatment of sinus structure abnormal headache has the feasibility and safety.
