Objective: To investigate the predictive value of serum CA125,HE4 combined with platelet/lymphocyte ratio(PLR),neutrophils/lymphocyte ratio(NLR),monocyte/lymphocyte ratio(MLR)in the diagnosis of epithelial ovarian cancer,in order to find an effective detection method,and to improve the diagnosis rate of early epithelial ovarian cancer.Methods: Retrospective analysis of 2014.01 to 2018.12 in the first hospital of lanzhou university,doctor of department of gynaecology cases,choose the first option surgical treatment and pathological results for epithelial ovarian cancer patients of 147 cases as observation group,there were 53 cases of early epithelial ovarian cancer in stage I and II of FIGO,and 94 cases of late epithelial ovarian cancer in stage III and IV of FIGO,at the same time the first clinical surgical treatment and pathological results in return for 150 cases of ovarian benign tumor patients as control group.General data,tumor markers(serum CA125,HE4)and blood routine indexes(platelet count,absolute value of neutrophils,absolute value of monocytes,and absolute value of lymphocytes)of the enrolled patients were collected,and PLR,NLR and MLR values were calculated.The differences of serum CA125,HE4,PLR,NLR and MLR in epithelial ovarian cancer and ovarian benign tumor,early epithelial ovarian cancer and late epithelial ovarian cancer,early epithelial ovarian cancer and ovarian benign tumor were analyzed.Results: 1.The contents of platelets,neutrophils,monocytes,lymphocytes,serum CA125,HE4,PLR,NLR and MLR in epithelial ovarian cancer group were significantly higher than those in the ovarian benign tumor group,The area under the ROC curve corresponding to the diagnosis of epithelial ovarian cancer by serum CA125 and HE4 combined with PLR,NLR and MLR was 0.941,and the sensitivity and specificity were 91.07% and 96.00%,respectively.The diagnostic accuracy,sensitivity and specificity were significantly higher than other combinations,and the difference was statistically significant(P<0.05);2.The contents of serum CA125,HE4,PLR,NLR and MLR in the late epithelial ovarian cancer group were significantly higher than those in the early epithelial ovarian cancer group,When serum CA125 and HE4 were combined with PLR,NLR and MLR in the diagnosis of early epithelial ovarian cancer,the corresponding area under the ROC curve was0.908,and the sensitivity and specificity were 90.36% and 80.85%,respectively.The accuracy,sensitivity and specificity of diagnosis were significantly higher than those of other combinations,and the difference was statistically significant(P<0.05);3.The contents of serum CA125,HE4,PLR,NLR and MLR in the early epithelial ovarian cancer group were significantly higher than those in the ovarian benign tumor group.The area under the ROC curve of serum CA125 and HE4 combined with PLR,NLR and MLR in the early diagnosis of epithelial ovarian cancer was 0.896,and the sensitivity and specificity were 89.81% and 94.67%,respectively.The diagnostic accuracy,sensitivity and specificity were significantly higher than other combinations,and the difference was statistically significant(P<0.05).Conclusion: Serum CA125,HE4,PLR,NLR and MLR have statistically significant differences in ovarian benign and malignant tumor,early and late epithelial ovarian cancer,early epithelial ovarian cancer and ovarian benign tumor,which are expected to be applied in epithelial ovarian cancer,and may have a hint and warning effect on the detection of early epithelial ovarian cancer. |
