Objective:To explore the related factors and efficacy analysis of lymphatic cyst occurrence and co-infection after radical resection of three common gynecological malignancies(cervical cancer,endometrial cancer and ovarian cancer).Method:A total of 351 patients with Gynecologic malignant tumor who were treated by undergoing laparoscopic surgery in the First Hospital of Lanzhou University from Jan 2017 to Jan 2019 were retrosoectively analyzed in the study,according to the presence of lymphocyst formation can be devided into lymphocyst group and no lymphocyst group,the two groups were compared in general condition,intraoperative status,postoperative laboratory tests and postoperatuve pathology.The20 patients with lymphocytic infection were divided into the single antibiotic group and the ultrasound-guided puncture and drainage group.Result:Single factor analysis showed that the removal of the number of lymph nodes(P=0.000),resection of lymph nodes(P=0.001),the drainage tube indwelling time(P=0.013),with and without postoperative radiotherapy(P=0.005),the patient's body mass index(BMI,P=0.000)and the level of three acyl glycerin gynecologic malignant tumor(P=0.004)is a risk factor for postoperative lymphocele,P<0.05,the difference was statistically significant;Logistic regression analysis showed that the number of lymph nodes removed during operation was an independent factor affecting the formation of lymphocysts(P<0.05),and the risk of lymphocysts increased by 0.995 times for every 10 additional lymph nodes removed(OR value was 1.995).The incidence of lymphadenopathy was 17.85%,and escherichia coli was the most common pathogen of lymphadenopathy,with an incidence of 31.2%.Univariate analysis showed that lymphatic infection and cyst diameter were closely related to diabetes mellitus(P=0.000).Logistic regression analysis showed that the cyst diameter(>5cm)was an independent factor for lymphocytic cyst infection(OR=4.375,P=0.041).Fisher's exact probability method was used to compare the efficacy of antibiotics alone and ultrasound guided puncture and drainage in the treatment of lymphatic infection.The difference was statistically significant(?~2=16.660,P=0.001).Conclusions:Related factors of gynecological malignant tumors after pelvic lymph node dissection include the number of lymph nodes removed,the extent of lymphectomy,the drainage tube retention time,postoperative adjuvant radiotherapy,the patient's BMI and triacylglycerol;Escherichia coli is the most common infection of lymph cysts,The diameter of common pathogenic bacteria is an independent risk factor for cysts with infection;Puncture and drainage combined with antibiotics have better results and can be used as their recommended treatment.
