Objective To evaluate the effect of ultrasound-guided erector spinae plane(ESP)block combined with patient controlled intravenous analgesia(PCIA)on postoperative analgesia and adverse rections in patient undergoing open hepatic lobectomy surgery. Methods Fifty-two patients(37males and 15 females, aged 24 ~77years,weight 47~ 80 Kg,ASA physical status ?or?),scheduled for elective open hepatic lobectomy from September 2018 to November 2019,were randomly assigned into two groups,ESP block combined with PCIA group(group PE)and PCIA only group(group P),26 patients in each group. In the PE group,a single erector spinae block was performed before anesthesia induction, and 0.4% ropivacaine 20 ml was given.VAS scores were recordde at 1,6,12,18,24 and 48h after operation. The frequency for compress PCIA,the volume of analgesia drugs, the days in hospital, the time to get out of bed for the first time and the side-effects were recorded as well. Results The 1-48 h resting VAS and coughing VAS scores in the PEgroup were lower than those in the P group(P <0.05).48 hours after block, the total number of compressions and the total number of compressions were significantly less than those in group P(P <0.05).The incidence of vomiting was lower than that in group P,but the difference was not statistically significant(P> 0.05).There was no statistically significant difference in the day of hospital stay and the time of first out of bed activity between the two groups of patients(P>0.05).Conclusion For patients with hepatic lobectomy,ultrasound-guided erector spinae plane block has clear analgesic effect and can also reduce the dose of intravenous analgesic drugs.
