The Effect And Time Of Inducing Labor Using Controlled-release Dinoprostone Vaginal Suppository In Term Pregnancy

Objective: To observe the effect and optimising daytime deliveries when inducing labor using controlled-release dinoprostone vaginal suppository(propess?)with various degrees of cervical ripening.Methods:The clinical data of patients who were induced labor with propess? from January 2012 to February 2019 in the Second Affiliated Hospital of Chongqing Medical University were analyzed retrospectively.Women were divided into three groups according to Bishop score before inducing labor.They were less than 3-point group,4-point group and 5-point group.Each group was divided into morning group,afternoon group,and night group by inducing timing.Results: 1139 pregnancies were included.The effective rate of cervical ripening was 93.6%,and in the less than 3-point group,4-point group and 5-point group was 95.8%,92.9% and 90.1%,respectively(P<0.05).The incidence of meconium-stained amniotic fluid,postpartum hemorrhage,fetal distress and postpartum hypertension was20.6%,2.2%,1.1% and 0.1%,respectively.The first two stages of labor in the night group were shorter than that in the morning group and afternoongroup in the less than 3-point group among primipara(P<0.05),while the first two stages of labor in the morning group and night group was shorter than that in the afternoon group in the less than 3-point group among multipara(P<0.05).There was no significant difference in other indicators between groups.Conclusion: Propess? is safe and effective for induction of labor in term pregnancy.Induction of labor could be started at night for multipara with cervical score less than 3 points and all primipara.For multipara with cervical score more than 3 points,the propess? should be placed when the medical staff is energetic and the women can be closely monitored.
