| ObjectiveTo provid some reference for the management about pelvic floor health of premenopausal women, comparing the clinical outcomes of premenopausal women with pelvic floor dysfunction(PFD)who chose different treatment methods(no treatment, physical therapy, surgical treatment).MethodsAt the gynecological outpatient service of the First Affiliated Hospital of Chongqing Medical University,41 cases of premenopausal women with PFD were selected into this study during the period from January 2017 to June 2018,and they were evaluated by pelvic organ prolapse quantitation(POP-Q),pressure test and pelvic floor impact questionnaire-short form 7(PFIQ-7).All cases were divided into the control group(10),physiotherapy group(11)and operation group(20).The objective efficacy of the control group was evaluated by POP-Q and pressure test at an interval of 1 year after the first diagnosis. In the physiotherapy group and the surgery group,it was evaluated at an interval of 1 year after the treatment. The subjectiveefficacy of three groups was evaluated with PFIQ-7 at intervals of 6 months and 1 year.Compare above indicators in each group and analyze the clinical outcomes of three groups.ResultsIn the objective effect of pelvic organ prolapse(POP),the aggravation rate of the control group was 75.0%.The improvement rate of the physiotherapy group was 66.7%.The cure rate of the operation group was100.0%.In the objective effect of stress urinary incontinence(SUI),the unhealed rate of control group was 100.0%.The cure rate and improvement rate of physiotherapy group were 30.0% and 30.0%.The cure rate and improvement rate of operation group were 53.8% and 46.2%.In the subjective effect of control group, the PFIQ-7 scores at 6 months and 1 year were higher than them at the first diagnosis, and the decline rate of life quality after 1 year was 75.0%.In the physiotherapy group and operation group,PFIQ-7 scores at 6 months and 1 year were all lower than them before treatment, and their improvement rates of life quality after 1 year were 54.5% and 88.9%.The differences of scores in every group were all statistically significant(P<0.05).ConclusionThe PFD in premenopausal women is clinically irreversible. If patients don’t take therapeutic measures, their objective symptoms and subjective feelings could not be alleviated, and their conditions willprogress to affect their life quality. The physical therapy and surgical treatment have objective and subjective effects for the PFD of premenopausal women. Therefore, during the development of PFD,it is necessary to take different therapeutic measures for all premenopausal female patients with different degrees of severity, delaying the progression of this disease, improving their life quality, and avoiding adverse clinical outcomes. |
