Background: There is no standard precaution for portal vein system thrombosis(PVST)after laparoscopic splenectomy in patients with cirrhosis.This work aims to determine the effect of terlipressin on PVST in patients with cirrhosis after laparoscopic splenectomy.Materials:We performed a retrospective cohort of patients with cirrhosis who received laparoscopic splenectomy from September 2000 to July 2019.A total of 174 consecutive cirrhotic patients were divided into two groups based on whether terlipressin was administered after operation on postoperative day(POD)1,2,and 3.Results: 52.87% patients(92/174)developed PVST(including portal vein thrombosis and spleen vein thrombosis.PVT and SVP)in our study.Although there were no significant differences in the overall incidence of PVST between Terlipressin(-)group(52.94%)and Terlipressin(+)group(52.84 %)(? 2=0.000,p= 0.989),the incidence of severe PVT(Grade 2 PVT and above)was significantly different between two groups(?2=13.907,p=0.000).The Child-Pughclassification scores on POD5 in Terlipressin(+)group was significantly improved compared with Terlipressin(-)group(p=0.020).We also found that the leucocyte counts on POD1 between the 2 groups was statistically significantly different(P=0.000),but not on POD5.Conclusion : These results demonstrate that treatment with terlipressin may reduce the incidence of severe thrombosis and improve liver function.What's more,terlipressin is likely to relieve postoperative infection in patients with cirrhosis after laparoscopic splenectomy.
