ObjectiveTo investigate the relationship between neutrophil-lymphocyte ratio(NLR)and Platelet-lymphocyte ratio(PLR)on severity and short-term prognosis of acute ischemic stroke.MethodsIn this study,192 patients with AIS admitted to the the Department of Neurology of the First Affiliated Hospital of Chongqing Medical University from October 2017 to February 2019 were enrolled.They were divided into different subgroups according to the MRI infarct size:the lacunar AIS group(<1.5cm)?medium-area AIS group(1.5-5.0cm)?the large AIS group(>5.0cm);according to the NIHSS score,it is divided into:mild group(NIHSS<4)?moderate group(4?NIHSS <15)and severe group(NIHSS?15);patients were divided into two groups according to the 90 d mRS score(0~2,3~6).Compare the levels of peripheral blood NLR and PLR between different subgroups,and evaluate the predictive value of thecombination of the two indicators on the short-term prognosis of acute ischemic stroke.ResultsThe NLR?PLR in the large AIS group was significantly higher than that in the lacunar AIS group(P<0.05).The NLR and PLR in the severe neurological deficit group were significantly higher than those in the mild group,and the difference was statistically significant(P < 0.05).The peripheral blood NLR and PLR levels of patients with poor prognosis were also higher than those with good prognosis.ROC curve analysis showed that the area under the curve(AUC)of NLR in the diagnosis of poor prognosis of AIS was 0.789,and its optimal cut-off value was 3.41,and the sensitivity was 86.3%,the specificity was 57.4%.Similarly,ACU of PLR was 0.672 and OOP was 187.06,which the sensitivity was 54.0% and the specificity was 75.2%.The sensitivity and the specificity of NLR and PLR parallel test were 88.2% and 53.2%,and the sensitivity and the specificity of NLR and PLR tandem test were 51.5% and 80.1%.ConclusionNLR ? PLR are closely related to the severity of acute ischemic stroke,can be used as an predictor of poor prognosis of AIS. |
