Objective:To explore the risk factors for bronchopulmonary dysplasia?BPD?accompanied by pulmonary hypertension?PH?in preterm infants,providing reference for clinical diagnosis and treatment.To identify and intervene early and timely,minimizing the risk of concurrent PH and improving the prognosis.Methods:The clinical data of 202 patients from January 2015 to December 2018 admitted to the neonatology department of our hospital,diagnosed as BPD(with oxygen dependence?oxygen absorption concentration?FiO2>21%?exceeding 28 days?were retrospectively analyzed,and their complete data,including general data,perinatal data,related clinical complications,oxygen therapy,cardiac color doppler ultrasound indicators,etc,were analyzed.The patients were divided into two groups,with PH group?n=79?and without PH group?n=123?,according to whether it is accompanied by PH.and then use univariate and multivariate logistic regression analysis,summarize the risk factors for BPD accompanied by PH.Results:There are no differences between the two groups of children in the proportion of gender,gestational age,birth weight,clinical outcome measures?P>0.05?,in comparison with the two group of the perinatal period data including the proportion of very immature,extremely low birth weight and low birth weight,twins/tire,1 min,and 5 min Apgar score,placenta previa,premature rupture of membranes,amniotic fluid dung,mother gestational age,pregnancy ICP,gestational hyperemia,there were no statistically significant difference in the two groups?P>0.05?;There were also no significant difference between the two groups in the rate of combination of pulmonary hemorrhage,apnea,NEC,ROP,IVH,non-invasive ventilation,and time of oxygen use?P>0.05?.While univariate analysis showed there were statistically significant differences between the two groups in the indicators of small for gestational age?SGA?,respiratory distress syndrome?RDS?,hemodynamic significant patent ductus arteriosus?hsPDA?,sepsis,moderate and severe BPD,invasive ventilation?7d?P<0.05?;multivariate analysis showed that RDS?OR=2.358,95%CI:1.2244.542,P=0.010?and hsPDA?OR=1.986,95%CI:1.0713.682,P=0.029?were independent risk factors for BPD accompanied by PH.Conclusion:RDS and hsPDA are independent risk factors for BPD accompanied by PH.For children with BPD,early identifying the risk factors and timely interveniong are necessary to minimize the risk of accompanied by PH.
