Objective: To investigate the risk factors of bronchopulmonary dysplasia(BPD)in preterm infants with respiratory failure. Methods: Retrospectively analyzed the clinical data of 212 preterm infants admitted to NICU of Children's Hospital of Chongqing Medical University from Jan 2016 to Dec 2018 with respiratory failure who were admitted within 7 days of age and hospitalized not less than 28 days. These preterm infants were classified into two groups according to whether BPD was complicated: BPD and non-BPD(control group). Results: The incidence of BPD in premature infants with respiratory failure was 39.6%(84/212).Univariate analysis showed that: gestational age, birth weight,Apgar score of 1 minute?7,neonatal respiratory distress syndrome(NRDS),coagulation dysfunction, ventilator associated pneumonia(VAP),periventricular-intraventricular hemorrhage(PIVH),use of pulmonary surfactant(PS)and the usage times, mechanical ventilation time were all correlated with a higher development of BPD(P < 0.05),no significant differences were found in incidence of septicemia, purulent meningitis, necrotizing enterocolitis(NEC),pulmonary hypertension(PH),pulmonary hemorrhage, air leak syndrome between the two groups. In the multivariate logistic regression model : gestational age, mechanical ventilation time and NRDS were independent influencing factors of BPD.Conclusions: Low gestational age, long duration of mechanical ventilation and combined with NRDS are the independent risk factors for BPD in premature infants with respiratory failure.
