Objective:To study the correlation between TCM syndrome elements and quality of life(QOL)in patients with rheumatoid arthritis(RA),to explore the distribution characteristics of TCM syndrome elements in RA patients and related factors affecting the quality of life of RA patients and to provide ideas for effectively improving the quality of life of RA patients clinically.Methods:Taking 283 patients with RA as research objects,the general clinical data,laboratory indicators,disease activity indicators and the total score of the MOS item short from health survey(sf-36)were collected,and the four diagnostic information of RA syndrome elements were collected according to the syndrome differentiation standards formulated by professor zhu wenfeng.SPSS 23.0 software was used for statistical analysis of clinical data.Research results:(1)The proportions of TCM syndromes in RA patients included in this study were mainly blood stasis,phlegm,dampness,cold and liver,accounting for 18.73%,16.96%,12.01%,9.89% and 9.18% of the total number of patients respectively.(2)The differences in the course of disease and employment of patients with each syndrome element RA were statistically significant(P<0.05).The differences in gender,age and education level were not statistically significant(P>0.05).(3)Through the analysis of Kruskal-wallish rank sum test of independent samples,it is found that the difference of SF-36 total score of different syndrome elements has statistical significance(P < 0.005).The scores of SF-36 patients with different syndrome elements RA in the dimensions of overall health(GH),physiological function(PF),physiological function(RP),body pain(BP),vitality(VT)were statistically significant(P <0.005).(4)The total score of Sf-36 of RA patients was generally lower.The differences of Sf-36 total scores in gender,disease course,employment status and other aspects were statistically significant(P<0.05),while the differences in age,education level and other aspects were not statistically significant(P<0.05).(5)Acording the Spearman correlation analysis and multiple linear correlation analysis showed that there was no significant correlation between anti-ccp titer,RF and SF-36 score(P >0.05).Syndrome element phlegm,blood stasis,yin deficiency and phlegm employment were significantly correlated with SF-36 total score(P< 0.05).Research conclusion:(1)The distribution of TCM syndromes of RA was dominated by blood stasis,phlegm,dampness,cold and liver.There were significant differences among TCM syndromes in disease course,employment status and anti-ccp titer.(2)The quality of life of RA patients generally decreased,and both physical health and mental health were affected.Female patients’ QOL was significantly lower than that of male patients.(3)There are differences in QOL of TCM syndromes of RA,among which QOL of syndromes of phlegm,blood stasis,liver and Yin deficiency is significantly lower than that of other syndromes.(4)The dimension of quality of life impairment of different syndrome elements is different.In the dimensions of body pain(BP)and vitality(VT),the quality of life of heat syndrome,blood stasis syndrome and dampness syndrome is significantly lower than that of other syndrome elements.(5)Syndrome element phlegm,blood stasis,liver,Yin deficiency and employment status are independent risk factors which affecting QOL. |
