Objective:Observing the effect of the abdominal penetrating moxibustion therapy on the muscle electrophysiology of transverse abdominal muscle and multifidus muscle of stroke patients by surface electromyography and evaluating the effect of the abdominal penetrating moxibustion therapy on postural control,balance,lower limb motor function by using relevant scales.Methods:Sixty-two stroke patients were randomly divided into a penetrating moxibustion group and a control group,and 58 cases were finally completed,29 cases in each group.The patients in the control group were treated with routine medical treatment,acupuncture treatment and rehabilitation training;the patients in the penetrating moxibustion group were additionally treated with penetrating moxibustion on abdominal.The abdominal penetrating moxibustion therapy was placed around Shenque,about 50 min each treatment.Both groups were treated 5 times a week for 4 weeks.The surface electromyography was measured before and after treatment at 4 weeks and Postural Assessment Scale for Stroke Patients(PASS),Berg Balance Scale(BBS),Fugl-Meyer Lower Limb Motor Function Assessment Scale(FMA)were evaluated before treatment,2 and 4 weeks after treatment.Results:(1)Root mean square of transverse abdominal muscle and multifidus muscle: Three movements included from sitting to standing,the unaffected side taking a step and the affected side taking a step after treatment,bilateral RMS of the penetrating moxibustion group were higher than those before treatment(P<0.01 in the affected side,P<0.05 in the unaffected side);bilateral RMS of the control group also increased(all P<0.05).After treatment,the RMS of affected side in the penetrating moxibustion group were higher than those in the control group(P<0.05),and there was no significant difference in the RMS of the unaffected side between two groups(P>0.05).Compared affected side with unaffected side,the unaffected side of the two groups were higher than the affected side from sitting to standing before treatment(all P<0.05).After treatment,there was no significant difference in bilateral RMS of the penetrating moxibustion group(P>0.05),while the unaffected side of the control group was higher than the affected side(P<0.05).When the unaffected side took a step,the unaffected side of the two groups were higher than the affected side before treatment(all P<0.05),and the differences between the affected side and the unaffected side of the two groups were not significant after treatment(all P>0.05).When the affected side took a step,the unaffected side of the two groups were higher than the affected side before treatment(all P<0.01).After treatment,the unaffected side of the penetrating moxibustion group was higher than the affected side(P<0.05),while the unaffected side of the control group was significantly higher than the affected side(P<0.01).(2)Median frequency of transverse abdominal muscle and multifidus muscle: Three movements included from sitting to standing,the unaffected side taking a step and the affected side taking a step after treatment,the MF of affected side and unaffected side in both groups were higher than those before treatment(all P<0.01 in the penetrating moxibustion group,all P<0.05 in the control group).The MF of affected side and unaffected side in the penetrating moxibustion group were higher than those in the control group(all P<0.05).Compared the difference of affected side and unaffected side of both groups patients before and after treatment,the MF of unaffected side were both higher than affected side of the movement from sitting to standing(all P<0.05).The MF of unaffected side was both significantly higher than affected side when the unaffected side took a step(all P<0.01).When the affected side took a step,the difference between the two sides were not significant(all P>0.05).(3)Scale score: Compared with those before treatment,the scores of PASS,BBS and FMA in both groups were improved after treatment at 4 weeks(all P<0.01).There was no significant difference in all indexes between the two groups after treatment at 2 weeks(all P>0.05).The scores of all scales in the penetrating moxibustion group were significantly higher than those in the control group after treatment at 4 weeks(P<0.01).Conclusion:The Abdominal Penetrating Moxibustion Therapy can significantly improve muscle strength and endurance of the transverse abdominal muscle and the multifidus muscle of stroke patients with hemiplegia and it also can improve the patients' postural control,balance and lower limb movement functions.
