Research background:Hypertension,as one of the most common chronic diseases of cardiovascular system,is often accompanied with dizziness and palpitations in the early stage,therefore,the control and treatment of Gastrodin is urgent.At present,gastrodin injection has been widely used in clinic as a treatment scheme for hypertension,but its specific mechanism and safety have not been clarified and verified,therefore,gastrodin injection is chosen as the research direction of this study.Meta-analysis method and network pharmacology are used to explore the clinical characteristics and mechanism of gastrodin injection in treating hypertension,it provides reference and direction for the clinical application of Gastrodin injection.Studyl:Meta-analysis of gastrodin injection for hypertension.Objective:To analyze and compare the safety and effectiveness of gastrodin injection combined with conventional western medicine and conventional western medicine in the treatment of hypertension,so as to provide reference for clinical rational use of drugs.Methods:In this study,the Chinese and English databases searched by computer include PubMed,EBSCO,the Cochrane Library,CNKI,VIP,Wan Fang data,etc.To obtain the randomized controlled trial(RCT)and observational study of gastrodin injection combined with conventional Western Medicine(test group)and conventional Western Medicine(control group)in the treatment of hypertension.According to Cochrane 5.1.0 evaluation manual,the quality of the included articles was evaluated,and then two researchers screened literature and extracted data according to the inclusion criteria,and then used Revman5.3 software to meta analyze the effective outcome evaluation indexes of the included articles.Results:A total of 67 articles were retrieved,and 11 articles were included in this s tudy,involving a total of 6101 patients,including 3058 patients in the experimental group and 3043 patients in the control group.The results of meta-analysis showed that gastroenterol injection combined with conventional western medicine was signifi cantly superior to conventional western medicine in terms of effectiveness,with stat istical significance[OR=3.15,95%CI(2.22,4.45),P<0.00001].In terms of blood pressure reduction,systolic blood pressure(SBP)[MD=-10.27,95%CI(-15.37,-5.18),P<0.00001],diastolic blood pressure(DBP)[MD=-6.16,95%CI(-9.47,-2.85),P=0.0003]and pulse press ure difference(PP)[MD=-6.02,95%CI(-8.12,-3.92),P<0.00001]combined with convention al western medicine showed more significant efficacy,and the difference was statistic ally significant.According to the analysis of vasoactive substances,nitric oxide(NO)[MD=6.86,95%CI(5.45,8.27),P<0.00001]and plasma endothelin(ET)[MD=-8.64,95%CI(-13.09,-4.19),P<0.00001]were also found in the experimental group.The safety assessment included dizziness[OR=0.39,95%CI(0.25,0.61),P<0.0001],nausea and vo miting[OR=0.51,95%CI(0.31,0.82),P=0.0006],cough[OR=0.44,95%CI(0.13,1.43),P=0.17]and fatigue[OR=0.43,95%CI(0.17,1.10),P=0.08].The incidence of cough and fati gue was also lower in the experimental group than in the control group.Conclusion:Gastrodin injection combined with conventional Western medicine group is effective in the treatment of hypertension,can significantly reduce the patients systolic pressure,diastolic pressure and pulse pressure difference,may be related to the regulation of vasoactive substances NO,ET,and higher safety,it is worthy of clinical use,but the quantity and quality of the articles need to be supported by more high quality Randomized controlled trial.Study 2:Network pharmacological study of gastrodin injection for hypertension.Objective:The single active component gastrodin in gastrodin injection,its target gene,pathway and biological process were analyzed by network pharmacology method,and the"component-target gene-pathway network pharmacological model was constructed,to predict the pharmacological action and corresponding pathway mechanism of Gastrodin injection in the treatment of hypertension,and to provide a new idea for clinical drug selection.Methods:Using Chemical Book,CTD,Swiss Target Prediction database access to basic information of gastrodine injection in gastrodia elata and Target genes,with the help of a STRING of platform construction of gastrodine injection Target genes interaction network,using GeneCards,CTD database for hypertension related Target genes,and use venny tool drawing gastrodine injection drug Target genes and Target genes of hypertension of Wayne figure,using Cytoscape 3.5.1 track of software builds active ingredient-Target gene network diagram analysis and topology parameters,Biological process enrichment analysis and genome encyclopedia pathway analysis were conducted on target genes through biological information annotation database DAVID,and finally the OmicShare cloud platform was used to visualize the pathway.Results:5441 hypertension genes were obtained from the database,among which 127 gastrodin genes were 101 common genes related to hypertension,and 19 key genes were further analyzed as TLR4,MAPK1,IL6,TNF,etc.A total of 306 pathways were obtained from KEGG pathway,among which 14 pathways were associated with hypertension with P<0.05,including renin angiotensin system signaling pathway,aldosterone regulating sodium reabsorption,and arachidonic acid metabolism pathway.According to GO functional enrichment analysis,328 biological processes were found,15 of which were involved in the regulation of hypertension,such as blood pressure regulation,relaxation of vascular smooth muscle,negative regulation of myocardial contraction,etc.Conclusion:In this paper,the pharmacological action and pathway mechanism of gastrodine injection in the treatment of hypertension were preliminarily discussed,which may be related to the involvement of 19 key target genes in 15 biological processes through affecting 14 hypertensive pathways,laying a foundation for further research.
