Obejective: Obstructive sleep apnea(OSA)could result in increase of Carotid intima media thickness(CIMT)and arterial stiffiness. Moreover,OSA is closely associated with adipcytikines, such as chemerin, adiponectin, secreted frizzled-related protein 5(SFRP5)and apelin. However, the association between adipocytokines and CIMT,arterial stiffiness in OSA patients is still unclear. Therefore, the goal of this study was to investigate whether adipocytokines play critical roles in the change of vascular function and structure. Methods: we recruited 95 participants without any comorbidity and distributed them in two groups based on their obstructive apnea-hypopnea index(AHI)(32 controls,63 OSAHS).All subjects underwent carotid artery ultrasound examination, polysomnography and blood sample was obtained to determine the serum chemerin,adiponectin,SFRP5,apelin levels. Results: The CIMT of OSAHS patients was significantly higher than that of the control group;the index of arterial stiffness such as distensiability coefficient(DC),compliance coefficient(CC)and pulse wave velocity(PWV)in OSAHS patients were significantly different from those in the control group.Serum SFRP5 in OSAHS patients was significantly lower than in the control group. There were no significant differences in serum adiponectin, chemerin and apelin level between the two groups.Correlation analysis showed that serum chemerin was positively correlated with BMI level;serum adiponectin level was negatively correlated with age, male percentage and triglyceride;serum SFRP5 level was negatively correlated with low density lipoprotein and AHI;serum apelin was only negatively ssociated with CT90.By stepwise regression analysis,we found that age was independently associated with CIMT and arterial stiffness. The minimum oxygen saturation was positively correlated with CC,negatively correlated with stiffiness coefficients ? and ?,and AHI was negatively correlated with DC,while serum chemerin, adiponectin, and SFRP5,apelin level was not independently associated with CIMT and arterial stiffness. Conclusion: OSAHS can increase the thickness of the carotid artery and the vascular stiffeness, thereby increasing the risk of atherosclerosis.Age,minimum oxygen saturation and AHI are the main influencing factors of arterial intima-media thickness and arterial elasticity. The relationship between serum chemerin,adiponectin,SFRP5,apelin levels and CIMT and arterial stiffness in OSAHS patients needs further study.
