Objective:To study the difference of serum irisin levels in patients with polycystic ovary syndrome(PCOS)with different body mass index(BMI)and to explore the changes of the serum irisin levels in overweight or obese PCOS patients before and after weight loss.Methods:1.Prospective study was used to analyze the clinical data of PCOS patients who first visited the Department of Reproductive Medicine of the Second Hospital of Hebei Medical University from February 2019 to December 2019.148 patients with PCOS were included,and the patients were divided into three groups according to BMI: 31 patients in PCOS normal weight group,60 patients in PCOS overweight group,and 57 patients in PCOS obesity group.Statistical analysis of the three groups of biochemical index,reproductive hormone index and irisin levels were performed using spss22.0 statistical software.2.After effective weight loss in some overweight or obese PCOS patients(at least 5% of their body weight),compare the changes in serum laboratory indexes and irisin levels before and after weight loss.Result:1.Comparison of general data: There were no statistical difference in age and height between the three groups(P>0.05).In the comparison of body fat rate,visceral fat level,waist circumference(WC)and waist hip ratio(WHR)indicators,the overweight group was higher than the normal weight group,and the obesity group was higher than the overweight group,and the differences were statistically significant(P<0.05).2.Comparison of laboratory indexes:(1)Comparison of liver function indexes:the levels of alanine aminotransferase(ALT)and aspartate aminotransferase(AST)in the obesity group and the overweight group were significantly higher than those in the normal weight group,and the difference was statistically significant(P <0.05).(2)Comparison of lipid metabolism indexes:the triglyceride(TG)level of the overweight and obesity group were significantly higher than that of the normal weight group,lipoprotein a(Lpa)and high density lipoprotein cholesterol(HDL-C)were significantly lower than the normal weight group,the difference was statistically significant(P <0.05),and there was no significant difference between the overweight and obesity group.The low density lipoprotein cholesterol(LDL-C)obesity group was higher than the overweight group,and the overweight group was higher than the normal weight group,but the difference was not statistically significant(P> 0.05).The apolipoprotein A(Apo A)obesity group was lower than the normal weight group,and the apolipoprotein B(Apo B)levels in the obesity group were higher than the normal weight group,with statistical differences(P <0.05).Total cholesterol(TC)level increased with the increase of BMI,but there was no statistically significant difference between the three groups(P> 0.05).(3)Comparison of glucose metabolism indexes:the levels of glycated hemoglobin(Hb A1C)and fasting insulin(FINS)in the overweight and obesity groups were significantly higher than those in the normal weight group,and the fasting blood-glucose(FBG)levels in the obesity group were higher than those in the normal weight group(P <0.05).(4)Comparison of reproductive hormones:the levels of anti-mullerian hormone(AMH)in the overweight and obesity group were significantly lower than those in the normal weight group,and the testosterone(T)level obesity group was higher than the normal weight group.The difference was statistically significant(P <0.05).The level of follicle stimulating hormone(FSH)and luteinizing hormone(LH)were not statistically significant(P>0.05).3.Comparison of irisin levels: the irisin concentration in the obesity group was significantly higher than that in the overweight and normal weight groups,and the serum irisin concentration was related to the patient 's BMI,body fat rate,visceral fat level,WC,WHR,Hb A1 C,ALT,AST FINS and T were significantly positively correlated(P <0.05).There was a significant negative correlation with Lpa(P <0.05).4.After effective weight loss,the serum irisin concentration,TC,TG,LDL-C,FBG,FINS,T in the overweight or obesity PCOS patients were significantly lower than those of weight loss before,Apo A,HDL-C were significantly higher than those of weight loss before,the differences were statistically significant(P <0.05).Conclusion:1.Overweight or obesity patients with PCOS have more serious endocrine and metabolic abnormalities than normal weight,weight loss is conducive to the improvement of endocrine and metabolic abnormalities,is the basis for the treatment of PCOS.2.Effective weight loss can reduce serum irisin levels and improve endocrine and metabolic disorders.
