| Objective:after the occurrence of both-column acetabular fractures,the two fracture lines divide the acetabular fractures into three major portions: the anterior column fragments,the posterior column fragments,and the iliac bone fragments adjacent to the axial skeleton.Because of the unique injury mechanism of both-column fractures,the displacement of anterior and posterior column fragments presents a certain regularity.In this study,the displacement characteristics of anterior and posterior column fragments were summarized by measuring and analyzing the separation points of each fracture fragment.Methods:This study selected 25 cases of both-column acetabular fractures in our hospital trauma emergency center from April 2017 to February 2019,pelvic-acetabulum CT original file import Mimics20(Materialise company,Belgium)software,After processing the data,the coordinates of the separation point of the injured anterior and posterior column fragments and the origin in the reversed copy of the healthy side were measured in the three-dimensional coordinate system,and the differences between the separation points and the origin were statistically analyzed.Results:The difference between the separation point of the injured anterior column fragment and the origin of the reversed copy in coordinate are X value: 5.38±4.28,P<0.05.Y value:-7.03±9.94,P<0.05.Z value: 9.5±7.40,P<0.05;The difference between the separation point of the injured posterior column fragment and the origin of the reversed copy in coordinate are X value: 16.64±4.34,P<0.05.Y value: 1.36±6.56,P>0.05.Z value: 6.44±5.64,P<0.05;The difference between the separation points of injured anterior and posterior column fragments in coordinate are X value:-10.55±4.91,P<0.05.Y value: 4.01±7.93,P>0.05.Z value: 1.88±3.17,P>0.05.Conclusions: The three-dimensional axial displacement of the anterior and posterior column fragments of the acetabular both-column fractures shows a certain regularity: the anterior column fragments are displaced inward backward and upward,the posterior column fragments are displaced inward and upward,and the posterior column is more displaced inward than the anterior column.A full understanding of the displacement characteristics of anterior and posterior column fragments can provide certain clinical guidance for the formulation of surgical approaches and the selection of internal fixations for patients with acetabular both-column fractures... |
