Objective: To investigate the characteristics of thoracic and lumbar motion in gait of patients with lumbar spinal stenosis(LSS)before and after decompression surgery.Methods: 47 LSS patients underwent decompression surgery in our hospital from January 2017 o March 2018 were selected as the study group,and 41 healthy population underwent physical examination in our hospital during the same period were selected as the control group.JOABPEQ was used to evaluate the patients' lumbar function before and after surgery,and VAS was used to evaluate the pain status before and after surgery.Gait Watch three-dimensional gait analysis was used to analyze the thoracolumbar motion characteristics of patients 6 weeks before and after surgery during standing and walking.Results: The scores of low back pain after operation were higher than those before operation,and the scores of low back pain VAS and leg pain VAS were lower than those before operation,with statistical significance(P < 0.05).There was no significant difference in the scores of lumbar function,walking function,social life and mental state between before and after operation(P > 0.05).The preoperative walking speed,step frequency and step length of the study group were lower than those of the control group,and the difference was statistically significant(P < 0.05).There was no significant difference in the Pace,pace frequency,stride length of the study group between postoperative and the preoperative indicators(P > 0.05).There was no significant difference in the indexes of trunk,thoracic vertebral curvature and pelvic inclination between the study group before operation and the control group(P >0.05).The extension of lumbar vertebrae in the study group was significantly reduced(P < 0.05),and there was no significant difference in the indexes of trunk,thoracic vertebrae,lumbar vertebrae curvature and pelvic tilt before and after LSS(P > 0.05).The correlation study showed that the maximum curvature of trunk was negatively correlated with leg pain VAS,and the sum of maximum thoracolumbar curvature was negatively correlated with leg pain VAS(r=-0.414 and-0.442,P < 0.05).Conclusion: Postoperative low back and leg pain of LSS patients were improved,and the above functional indicators of the study group still had a certain gap with normal people in terms of Pace,pace frequency,stride length;the postoperative lumbar spine extension and maximum curvature were improved in the study group,which has some correlation with patient leg pain.
