Objective: To evaluate the clinical effect of comprehensive therapy of supplementing qi,expelling dampness, and dissipating blood stasis on recurrent pelvic inflammatory disease(PID)patients with qi deficiency and blood stasis with dampness. To explore its regulation of high-sensitivity Creactive protein(hs-CRP),and soluble urokinase-type Plasminogen Activator Receptor(su PAR)tumor necrosis factoralpha(TNF-?)expression.Methods: Totally 100 recurrent PID patients(Qi deficiency and blood stasis with dampness)were selected as the research objects, and 30 cases of safetyobservation and evaluation were selected according to the random number table method.(1)It was treated for 3 months with the comprehensive schemeof supplementing qi,expelling dampness, and dissipating blood stasis(decoction of traditional Chinese medicine, decoction of banyan and rehabilitation, enema with retention of gynecological enema and moxibustion on of Zhongwan,Shenque,Guanyuan and Zusanli points).TCM syndrome score, pelvic physical sign score and pelvic pain score were used as therapeutic evaluation indexes.TCM syndrome score and pelvic physical sign score were conducted every 2 weeks and every 4 weeks separately. After 12 weeks of treatment, the symptoms, physical signs and pelvic pain scores of the patients were contrasted with before the treatment and the clinical effect was evaluated. The patients were followed up 12 weeks after drug withdrawal, and the recurrence was observed and evaluated.(2)The fasting blood samples were collected within 7 days before and after treatment for the following tests: hs-CRP(immunoturbidimetry),su PAR(enzyme-linked immunosorbent assay),TNF-?(chemiluminescence).Results:1.Clinical observation results:1.1 Analgesic effect:the total effective rate was 97.00%,the cure rate was 19.00%,the remarkbable effec rate was 53.00%,the significant and effective rate was 72%,the effective rate was 25%.Before treatment, the score of pelvic pain was 4.94±1.25,and after treatment, the one was 1.78±0.94,which was 3.16±1.50 lower than before. The difference is of statistical significance(P < 0.05).1.2 Physical signs: the total effective rate was 97.00%,the cure rate was 3.00%,the remarkbable effec rate was 63.00%,the significant and effective rate was 66%,the effective rate was 31%.The score of physical signs before treatment was 9.22±2.46,and the score of physical signs after treatment was 2.89±1.58,which was 6.33±2.74 lower than before. The difference is significantly of statistical significance(P < 0.001).1.3 Curative effect of TCM syndrome: the total effective rate was 97.00%,the cure rate was 0.00%,the remarkbable effec rate was 79.00%,the significant and effective rate was 79%,the effective rate was 18%t.Before and after treatment, the scores of TCM syndrome efficacy were 15.76±3.01,5.46±2.32 seperately. The difference between before and after is 10.30± 4.12,which was significant difference between before and after treatment(P < 0.001).1.4 Effect of single symptom: single symptoms were relieved after the treatment, the difference is of statistical significance(P < 0.05).1.5 Single sign effect: the difference of single sign effect before and after treatment was statistically significant(P < 0.05).1.6 Recurrence rate: 72 patients who recovered from pelvic pain after treatment were followed up for 12 weeks.1.7 Safety evaluation: there were 3 cases of mild adverse events,1 case of urinary tract infection,1 case of epigastralgia,1 case of diarrhea,no serious adverse events.2.Laboratory index test results:2.1 Cytokine index: the expression levels of su PAR,hs CRP and TNF-? decreased comparing ones before treatment, the difference of which was of statistical significance(P < 0.05).Conclusions:1.The comprehensive scheme of supplementing qi,expelling dampness, and dissipating blood stasis interferes with PID patients(Qi deficiency and blood stasis with dampness),obviously improves the symptoms of pelvic pain, pelvic physical signs and syndromes of traditional Chinese medicine. It has good clinical effect, and the treatment is safe, without obvious adverse reactions. The recurrence rate is low after 12 weeks of discontinuation, which is suitable for clinical promotion.2.Yiqi Qingshi Huayu therapy can reduce the expression of inflammatory cytokines such as su PAR,hs CRP and TNF-? in peripheral blood, improve immune function and reduce inflammatory reaction.
