| Objective:The effectiveness and safety of the Compound Kushen injections combined with cisplatin intraperitoneal infusion for malignant ascites were analyzed.The clinical application of Chinese medicine injections combined with chemotherapy was explored.The study provided evidence-based medical evidence for the evaluation of Chinese medicine injection combined with chemotherapy intraperitoneal infusion for the treatment of malignant ascites,reducing adverse reactions and optimizing treatment effect.Methods:Comprehensive retrieval of Chinese and English databases from Wanfang Data,VIP Chinese Journal Database(VIP),China Biomedical Literature Service System(Sino Med),China Knowledge Network Database(CNKI),The Cochrane Library,Embase and Pub Med,and other relevant randomized controlled clinical research literature.Strict literature selection based on inclusion and exclusion criteria,literature quality assessment using bias risk assessment tools,and extraction of data on outcomes such as recent efficiency,quality of life assessment,safety assessment,and immune function included in the literature.The data were tested for heterogeneity and metaanalysis of each outcome index by Stata 15.1.Results:The study included a total of 17 RCT studies with a total of 1382 patients,including 690 in the treatment group and 692 in the control group.The outcome indicators were analyzed and evaluated.The results are as follows:(1)The comparison of the effectiveness of the main outcome indicators showed that the improvement of the effectiveness of Compound Kushen injection was more obvious [OR=3.43,95%CI(2.68,4.39)],which was more advantageous than the cisplatin group,and the advantage was not It is affected by the dose and course of different drugs.Subgroup analysis shows the dose of the drug [OR=3.43,95%CI(2.68,4.39)],the course of treatment [OR=3.43,95%CI(2.68,4.39)];(2)Nine studies evaluated quality of life status.Analysis results showed that Compound Kushen injection can significantly improve patients' quality of life [OR=3.91,95%CI(2.75,5.54)],and improve KPS scores [WMD=10.97,95%CI(9.09,12.85)];(3)Among the study of the indicators of toxic and side effects,eight studies have reported myelosuppression.The incidence of myelosuppression in the Compound Kushen injection group was lower than that in the single drug cisplatin group [OR=0.40,95%CI(0.28,0.57)].Ten studies reported nausea and vomiting results,showing that the incidence of nausea and vomiting in the Compound Kushen injection group was significantly lower than that in the single-dose cisplatin group [OR=0.34,95%CI(0.24,0.48)],and subgroup analysis suggested drug dose [OR=0.34,95%CI(0.24,0.48)],course of treatment [OR=0.33,95%CI(0.33 0.49)].Three studies reported on abdominal pain and diarrhea.Compound Kushen injection can reduce abdominal pain and diarrhea.Occurred [OR=0.25,95%CI(0.11,0.56)].Four studies reported fever and fatigue,and Compound Kushen injection could reduce the incidence of fever and fatigue [OR=0.41,95%CI(0.22,0.78)].Four study reported liver and kidney damage.The liver and kidney damage of the Compound Kushen injection group was lower than that of the cisplatin group alone [OR=0.51,95%CI(0.30,0.86)];(4)A total of five studies have reported on immune indicators.CD3+ [WMD=6.59,95%CI(2.29,10.89)],CD4+ [WMD=7.03,95%CI(3.39,10.67)],CD4+ / CD8+ ratio [WMD=0.33,95%CI(0.12,0.54)].The Compound Kushen injection combined with cisplatin has more advantages than cisplatin alone in improving the immune function of patients with malignant ascites,and it is not affected by the dose and course of treatment.Conclusion:(1)The use of Compound Kushen injection in patients with malignant ascites can improve the treatment efficiency and improve the quality of life,and the effect is not affected by the dose and course of treatment.(2)Compound Kushen injection has obvious advantages in effectively reducing the occurrence of bone marrow suppression,nausea and vomiting,abdominal pain and diarrhea,fever and fatigue,and reducing liver and kidney damage. |
