Objective To observe the effect of dexmedetomidine on the level of oxidative stress and related indexes of renal function in patients with intracranial aneurysm during clipping operation.Methods From January 2019 to November 2019,60 patients who underwent selective middle cerebral artery aneurysm clipping were randomly divided into right dexmedetomidine group(D group,n=30)and control group(C group,n=30).Ten minutes before anesthesia induction,group D received intravenous infusion of 0.8?g/kg/h loading dose of dexmedetomidine,followed by intravenous pump of 0.5?g/kg/h until the end of the operation.The control group was given the same volume of saline intravenous pump until the end of the operation.The age,sex,body mass index,preoperative complications,anesthesia time,the use of vasoactive drugs and the amount of surgical access were recorded.The mean arterial pressure(MAP),heart rate(HR),blood glucose,BIS and urine volume were recorded at 30 minutes before anesthesia induction(T0),after dural incision(T1),after operation(T2),24 hours after operation(T3)and 72 hours after operation(T4).The levels of serum creatinine(SCR),urea nitrogen(BUN),malondialdehyde(MDA),superoxide dismutase(SOD)and cystatin C were measured at the above five time points.Results 1 The dosage of sodium nitroprusside and propofol in group D was observably lower than that in group C(P<0.05).2 HR and urine volume of the two groups at T1 and T2 were higher than that at T0,the distinction was observably crucial(P<0.05);At T1 and T2,HR of the D group was observably inferior than that of the C group,and urine volume of the D group was greater than that of the C group,the distinction was observably crucial(P<0.05);Due to the use of controlled hypotension during operation,there was no statistically significant difference in map at each time point(P>0.05).3 At the time of T2-T4,the MDA and SOD level of the two groups increased gradually,and declined,the distinction was observably crucial(P<0.05);At the time of T2-T4,the MDA of the D group was inferior than that of the C group,and the SOD level was greater than that of the C group,the distinction was observably crucial(P<0.05).4 At the time of T1-T4,the SCR and bun of the two groups increased slowly,but there was no statistical significance;At the time of T1-T4,the SCR and bun of the D group were mildly inferior than that of the C group.There was no meaningful difference between the two groups.5 From T1 to T4,the cys-C concentration of the two groups increased meaningful(P<0.05);and the cysC concentration of the D group was inferior than that of the C group(P<0.05).Conclusion There is a trend of acute renal injury in the perioperative period of aneurysm clipping operation.The application of dexmedetomidine in the perioperative period can significantly reduce the level of oxidative stress in patients,protect the renal function of patients,and improve the life support after treatment.Figure 10;Table 6;Reference 120...
