Objective: In this study,the pathological characteristics,treatment methods and postoperative adjuvant therapy of 58 patients with ovarian borderline tumor who were initially treated with surgery in our hospital were statistically analyzed to study the influencing factors of postoperative recurrence of ovarian borderline tumor,so as to provide reference for the treatment of ovarian borderline tumor.Methods:The clinical and pathological data of 58 cases of borderline ovarian tumors treated in the Second Hospital of Hebei Medical University from January 2003 to December 2017 were summarized.SPSS 25.0 software was used for statistical analysis.Univariate analysis was performed using chi-square test or exact probability method.Multivariate analysis was performed using Logistic regression model.P<0.05 was considered statistically significant.Results:A total of 58 patients with borderline ovarian tumors were included in this study.The average age of onset was 43 years,and the median follow-up time was 78.5 months.A total of 14 patients relapsed during the follow-up period,with a recurrence rate of 24.14%.In the choice of surgical approach,the postoperative recurrence rate and the maximum tumor diameter of laparotomy were higher than those of laparoscopic surgery,and the differences were statistically significant(P=0.043,P<0.001).The diameter of mucinous tumors was significantly larger than that of serous tumors.In patients,the risk of tumor rupture increases during surgery.Multivariate analysis showed that tumor residue was an independent risk factor for postoperative recurrence in patients with BOT,and the risk of postoperative recurrence increased by nearly 5 times(P=0.036<0.05,OR=4.922).The postoperative recurrence rate of patients with FIGO late stage and patients with extraovarian lesions was 55.56%,which was significantly higher than other patients,and the results were statistically significant(P=0.030).In addition,a total of 31 patients underwent fertility-preserving surgery in this study.The postoperative recurrence rate was 32.26%,which was slightly higher than 14.81% of patients undergoing radical surgery,but the difference was not statistically significant.Due to the short follow-up time and the small amount of case data collected,further follow-up evidence is needed.Conclusions:1.In the late stage of FIGO and large tumor diameter,laparotomy is recommended.Laparoscopic surgery can be used for early young patients,but patients with a tumor diameter greater than 10 cm have a risk of tumor rupture.It is recommended to switch to open surgery to avoid postoperative tumor remnants.2.Residual tumors,FIGO stage,and extraovarian lesions are the influencing factors of postoperative recurrence in BOTs patients.Among them,residual tumors are independent risk factors for postoperative recurrence in BOTs patients.3.For patients with fertility requirements,conservative surgical treatment is safe and feasible;ovarian tumor removal or appendectomy does not increase the risk of postoperative recurrence.When choosing a surgical method,in order to retain more ovarian tissue,ovarian tumor removal is feasible.However,care must be taken to prevent tumor rupture.4.It has not been found that adjuvant chemotherapy after BOTs can reduce the recurrence rate...
