OBJECTIVE: To analyze the physical examination results of people undergoing physical examination in our hospital,and to understand the distribution of risk factors related to cardiovascular disease and the stratification of disease risk.Make a telephone questionnaire for ASCVD high-risk groups to understand their transitional care after physical examination,understand the development of related continuous medical services and its influencing factors,and provide scientific basis for future cardiovascular disease prevention and treatment.Methods: Collected the people who had undergone a health checkup at the Health Promotion Center of our hospital from January 2019 to June 2019 and kept the complete results. Filtered the risk assessment of ASCVD is high-risk and extremely high-risk groups,and a telephone questionnaire is conducted on the medical treatment after medical examination,and the results of the telephone questionnaire are analyzed.Results: A total of 17,560 people were included in the examination,including 9567 men(54.5%)and 7993 women(45.5%).There were 1129 high-risk ASCVD patients,including 786 males(69.6%)and 343 females(30.4%).The average age was 54.91 ± 10.05 years.A total of 878 valid telephone questionnaires were obtained,including 613 males and 265 females,with an average age of(54.56 ± 10.32)years.The prevalence of hypertension in the general population was 14.6%,which was higher in men than in women(17.8% VS 10.8%,p <0.001),and the prevalence increased significantly with age;the prevalence of diabetes was 4.1%,and men were higher than women(5.2% VS 2.7%,p <0.001),and the prevalence increased significantly with age;the prevalence of hyperlipidemia was 5.6%,which was higher in men than women(7.3% VS 3.6%,p <0.001),and The prevalence increased significantly with age;the obesity rate was 10.8%,and men were higher than women(15.3% VS 5.5%,p <0.001),and the obesity rate was increasing with age;the smoking rate was 0.7%,and men Higher than women(1.4% VS 0%,p <0.001),there was no significant difference in smoking rates between different age groups.The abnormal blood pressure control rate is 15.4%,of which 50.1% of patients who have previously had hypertension have abnormal blood pressure;the abnormal blood glucose control rate is 3.2%,of which 46% have been previously diagnosed by patients with diabetes;abnormal blood lipid control rate is 32.3 %,Of which 100% have dyslipidemia in patients with previous hyperlipidemia.Blood pressure control failure rate in high-risk ASCVD population was 34.5%,men were lower than women(32.6% VS 39.1%,p = 0.035),and blood pressure control failure rate increased significantly with age;blood glucose control failure rate was 29.8%,Males were higher than females(31.6% VS 25.7%,p = 0.046),and the rate of blood glucose control failed to increase with age;the rate of blood lipid control failed to reach 60.9%,there was no significant difference between men and women(p = 0.272),and with the decline of age,the rate of failure to control blood lipids increased significantly;the obesity rate was 17.5%,and men were higher than women(20.7% VS 10.2%,p <0.001).Obesity rates vary among different age groups,of which 40-50 The obesity rate was highest in the age group;smoking rate was 2.3%,and men were higher than women(3.3% VS 0%,p = 0.001).There was no significant difference in smoking rates among different age groups.The awareness rate of disease in high-risk patients with ASCVD is 82.6%,and the awareness rate declines significantly with age.The medical examination reports of 769(87.6%)patients were interpreted by doctors,and 720(82%)patients were informed of high cardiovascular risk In the population,49(5.6%)patients were not informed,109(12.4%)patients' medical reports were not interpreted,a total of 710(80.9%)patients could be followed up regularly,and 381(53.7%)were followed up in community hospitals,The follow-up rate showed a significant decline with age.More than 97% of the follow-up patients were able to comply with "follow-up,drugs,monitoring,smoking cessation and alcohol restriction",82.39% of patients could eat reasonably,and only 41.69% of patients could follow "reasonable exercise".Conclusion: The exposure rate of hypertension,hyperglycemia,hyperlipidemia,and obesity is higher among the people undergoing physical examination in our hospital.,Men are significantly higher than women,and they are increasing with age.Patients who have identified underlying diseases,Poor control of risk factor levels,especially dyslipidemia;the implementation of transitional services after physical examination in high-risk ASCVD patients is generally acceptable,and most patients can be fully,continuously,and effectively managed.In the management process,special attention needs to be paid to the young patients with high risk or dyslipidemia,and increase their awareness to promote regular follow-up.
