Objectives This study describes the current state of pregnancy-related anxiety in primiparas during early pregnancy,analyzes the influencing factors of pregnancy-related anxiety and its correlation with family function and social support.At the same time,this study also discussed the influence of family support intervention on the level of pregnancy-related anxiety,family function and social support in primiparas.Methods This study consists of two parts.(1)Part I: Study on the influencing factors of pregnancy-related anxiety and its correlation with family function and social support in early pregnancy.This study is a cross-sectional study.According to the inclusion and exclusion criteria,773 pregnant women(£13+6 week)who were registered in the obstetrical outpatient department of affiliated hospital of Inner Mongolia medical university from march to December 2019 were selected as the study subjects.Survey tools include: General information questionnaire,Pregnancy-specific anxiety questionnaire(PAQ),Family concern index questionnaire(APGAR),Perceived social support scale(PSSS).All the analysis were conducted using SPSS20.0.Applied statistical methods include Chi-square test,Spearman correlation analysis,Binary logistic regression analysis.(2)PartⅡ: Family support intervention for primipara with pregnancy-related anxiety.This study is a quasi-experimental study.According to the inclusion and exclusion criteria,112 primiparas and their spouses who were positive for pregnancy-related anxiety(PAQ ≥ 24)were selected as the research objects.The experimental group and the control group each had 56 samples.From the first part of the study,we can know that family function can be changed through intervention.Therefore,based on psychological stress theory,we formulate a program to intervene in families with pregnancy-related anxiety.The control group received routine care,while the experimental group received family support intervention on the basis of the control group,including We Chat group information support,on-site one-to-one guidance and "family group activities for pregnant women".Reassessment of pregnancy-related anxiety,family function,and social support in primiparas after 10 weeks of intervention.Compare the differences in pregnancy-related anxiety,family function,and social support between the experimental group and the control group before and after intervention.And analyze pregnancy-related anxiety,family function,and social support before and after intervention in the experimental and control groups,respectively.Applied statistical methods include Chi-square test,Mann-whitney test,Wilcoxon symbolic rank test.Results(一)Part I findings(1)The median pregnancy-related anxiety score of 790 primiparas was 19.0,of which 154 women had a PAQ of 24 and the median score was 26.5.The incidence of pregnancy-related anxiety was 19.9%.(2)Multivariate Logistic regression results show: The risk of pregnancy-related anxiety in primiparas during early pregnancy with physical discomfort was 1.836 times higher than that in women without physical discomfort(OR=1.836,95%CI:1.183,2.850);The risk of pregnancy-related anxiety in primiparas during early pregnancy with high work stress was 3.046 times higher than that in women with less work stress(OR=3.046,95%CI:1.487,6.242);In the past six months,the pregnancy-related anxiety of primiparas with major family accidents was 3.280 times higher than that of primiparas without major family accidents(OR=3.280,95%CI:1.173,9.175);The risk of pregnancy-related anxiety in primiparas with general physical condition before pregnancy is 2.234 times higher than that in healthy condition(OR=2.234,95%CI:1.417,3.523);The risk of pregnancy-related anxiety in primiparas with unintended pregnancies is 3.126 times that of those who are fully prepared(OR=3.126,95%CI:1.365,7.160);The risk of pregnancy-related anxiety was 2.746 times(OR=2.746,95% CI:1.676,4.500)and 4.977 times(OR=4.977,95% CI:1.457,17.001)higher in primiparas with moderate and severe family dysfunction than in those with good family function.(3)There was a negative correlation between pregnancy-related anxiety and family function(P(27)0.01).The family function score was positively correlated with the total social support score and its family support dimension,and the score of external family support dimension(P(27)0.01).(二)Part two findings A total of 97 primiparas and spouses completed the study,49 in the experimental group and 48 in the control group.After the implementation of family support interventions,the total scores of pregnancy-related anxiety and the dimensions of fear of fetal health in the experimental group were significantly lower than those in the control group(P(27)0.01),and the total scores of family function and their fitness and cooperation in the experimental group were scored.The scores were significantly higher than those in the control group(P(27)0.01).The scores of the total scores of social support,family support,and out-of-home support in the experimental group were higher than those in the control group(P(27)0.05).The total scores of pregnancy-related anxiety after the intervention in the experimental group The scores of each dimension were significantly lower than the scores before intervention(P(27)0.01).The scores of the total family function scores,their fitness,cooperation,and affection scores after the intervention were significantly higher than those before the intervention(P(27)0.05).The scores of total social support scores and their family support dimensions were higher than those before the intervention(P(27)0.05);the total scores of pregnancy-related anxiety after intervention and their scores in the control group were lower than those before the intervention(P(27)0.01),and family function after the intervention The scores of the two dimensions of cooperation and maturity were higher than the scores before the intervention(P(27)0.05).The scores of the total family function before and after the intervention,as well as the three dimensions of fitness,affection,and intimacy,and the total score of social support.Its various dimensions score difference was not statistically significant(P(29)0.05).Conclusion Pregnancy-related anxiety is more common in early primipara.Poor physical condition before and during pregnancy,major family accidents in the past half a year,high work pressure during pregnancy,unexpected pregnancy and family dysfunction are the risk factors of pregnancy-related anxiety in primiparas.There was a negative correlation between pregnancy-related anxiety and family function in primiparas during early pregnancy,the family function score was positively correlated with the total social support score and its family support dimension,and the score of external family support dimension.Family support interventions can help improve the family function and social support of primiparas,and help alleviate pregnancy-related anxiety in primiparas. |
