Objective:Compare the expression of heat shock protein 27(HSP27)in different types and stages of lower extremity arteriosclerosis occlusive disease(LEASO)patients,and compare 3 normal femoral artery samples(normal group)and 3 atherosclerotic femoral artery samples(Pathological group)HSP27 and Phospho-HSP27(Ser15),Phospho-HSP27(Ser78)and Phospho-HSP27(Ser82)expression differences,and explore their clinical significance in the pathogenesis and treatment of LEASO.Methods:A total of 60 LEASO patients from our hospital from September 2017 to December 2018 were prospectively included in the hospital.And clinical data of 3patients with severe amputation due to LEASO gangrene.Sixty LEASO patients completed serum collection before surgery.The patients were divided into two groups according to the size of the ankle-brachial index(ABI): group A(ABI?0.43)and group B(ABI > 0.43),30 cases in each group(ordered according to the ABI size of the patient,0.43 was the median).Enzyme-linked immunosorbent assay was used to complete the detection of HSP27 in all samples.The expression of HSP27 in each stage of Fontaine was observed,and the differences in HSP27 concentration and ABI between the two groups of patients were compared.The femoral artery specimens of 6 patients with amputation were used to detect HSP27 and its 3 phosphorylation sites by Western blotting.The expression differences between the normal group and the diseased group were compared.Results : Fontaine stage I,stage IIa,stage IIb,stage III,and stage IV HSP27 concentrations were(92.14 ± 5.61)ng / ml,(68.55 ± 18.19)ng / ml,(55.31 ± 4.68)ng /ml,(41.74 ± 12.09)ng / ml,(25.00 ± 4.39)ng / ml.The HSP27 concentration in group A was(40.73 ± 15.99)ng / ml,and the ABI was 0.26 ± 0.20;the HSP27 concentration in group B was(66.30 ± 24.70)ng / ml,and the ABI was 0.64 ± 0.20.The concentration of HSP27 in group B was significantly higher than that in group A(P <0.05).The relative expression levels of HSP27 and Phospho-HSP27(Ser15),Phospho-HSP27(Ser78)and Phospho-HSP27(Ser82)in the normal group were 0.82 ± 0.13,0.66 ± 0.12,0.91 ± 0.24,0.90 ± 0.16 respectively;The relative expressions were 0.45 ± 0.08,0.42 ±0.09,0.39 ± 0.12,0.58 ± 0.11.The relative expression levels of HSP27 and its three phosphorylation sites in the normal group were significantly higher than those in the diseased group(P <0.05).Conclusion:In patients with LEASO,the higher the Fontaine stage,the lower the ABI,the lower the HSP27 concentration;conversely,the lower the Fontaine stage,the higher the ABI,the higher the HSP27 concentration.Serum HSP27 concentration was negatively correlated with LEASO severity,and HSP27 concentration was positively correlated with ABI value.The content of HSP27 and its phosphorylation in the femoral artery of normal people was significantly higher than that of femoral artery in patients with atherosclerosis.Elevated levels of HSP27 and its phosphorylation in humans can inhibit the progression of LEASO disease.The study of HSP27 is of great significance to the pathogenesis,assessment and treatment of LEASO.
