Objective: Hepatitis E caused by Hepatitis E virus(HEV)infection is mostly acute self-limiting hepatitis,and severe hepatitis rarely occurs.Once it occurs,the patient's condition can develop rapidly,with multipleorgan dysfunction or even death in the short term.Patients with severe hepatitis are susceptible to secondary infection due to damage to immunefunction and intestinal microecological disorders.Antibacterial drugs are an effective means of controlling infection,and its long-term application has aggravated the degree of intestinal microecological disorders.Antibacterial drugs and intestinal microecological disorders are also important reasons in the occurrence of Antibiotic-associated diarrhea(AAD).Therefore,this article explains the risk factors of AAD,the mechanism of intestinal microecological disorders,diagnosis,reatment and prevention,in order to improve clinicians' understanding of it,and provide help for clinical diagnosis and treatment of similar diseases.Methods: Select the patient's clinical datas who had acute severe hepatitis E with AAD,the patient hospitalized in Infection Disease Deparment of the Third Hospital of Hebei Medical University on 2017.By relevanting literatures at home and abroad and summarize the risk factors of AAD,the mechanism of intestinal microecological disorders,diagnosis,treatment and prevention.Results: In this case,HEV infection resulted in severe hepatitis and extrahepatic damage.At the same time,AAD appeared after the application of multiple antibacterial drugs due to secondary infection.After treatment of primary disease,secondary infection and AAD,his condition improved.Conclusions: The long-term application of antibacterial drugs and the resulting intestinal microecological imbalance are the main factors and mechanisms leading to AAD.The treatment of AAD focuses on stopping antibacterial drugs and restoring the intestinal microecological balance.AAD should strive for early detection,early diagnosis,and early treatment,and appropriate treatment measures should be taken in time to avoid more serious complications.
