Background: Vascular cognitive impairment no dementia(VCIND)has a high rate of progression to dementia,and studies have proven that intervention can delay its progress.Dl-3-n-Butylphthalide(NBP)can improve cognitive impairment,but its role in brain cognitive structural mechanism is not clear.Objective: To dynamically observe the specific cognitive domain change pattern of dl-3-n-butylphthalide in the treatment of VCIND and discover the neuroimaging mechanism of dl-3-n-butylphthalide intervention in the early stages of affecting brain cognitive related structures.Methods: Sixty patients with VCIND were selected and randomly divided into experimental group(600mg / day)and placebo group(60mg / day).MMSE,Mo CA and CDR scales were performed at baseline,1st,3rd,and 6th month,and Simultaneously functional magnetic resonance data was collected,to dynamically analyze the differences of each cognitive domain between the groups and compare the longitudinal changes of the cognitive structure of the brain.Results: 1.The cerebellar volume of both groups increased after 1 month,and the change rate of the NBP experimental group was more significant;2.Repeated ANOVA results of the MMSE and Mo CA scales display no statistically significant difference between the group,time point and the time point×group interaction(P>0.05),which means there was no significant change trend between MMSE and Mo CA in the two groups with time point;the results of the subscale showed that,compared with baseline,the attention of the 1st and 6th months increased in NBP group,and the 6th month in the placebo group increased compared with the month(P<0.05).Compared with baseline,visual space and execution in NBP group increased in the 6th months and recall ability increased in the 1st,3rd,and 6th months(P<0.05),and delayed recall increased in the 3rd and 6th months(P<0.05),and the placebo group also increased at the 6th month(P<0.05).There was no significant difference in language and abstract ability.Conclusions:Butylphthalide can increase cerebellar volume to improve cognitive function,and Simultaneously it can improve visual space and execution,attention,recall ability and delayed recall ability.
