Objective: To investigate the differences in the serum lipid profile in the severity of cirrhosis and the role of early prognosis prediction in patients with liver cirrhosis and portal vein thrombosis.Methods:Ninety-three patients with liver cirrhosis and portal vein thrombosis were included in the study.There were 16 patients in the non-survival group and 77 patients in the survival group according to the 2-year prognosis.There were 18 cases of CTP grade A,55 cases of CTP grade B,and 20 cases of CTP grade C.There were 46 cases of MELD <10 points and 47 cases of MELD ?10 points.The differences in lipids and other parameters were compared among the groups.Further correlation analysis and survival analysis were performed.Results:1.There were significant differences in PLT,CHE,PT,PA,INR,HDL-C,CTP score and MELD score between the non-survival group and the survival group.There were no significant differences in comparisons between age,sex,etiology,and splenectomy.Multifactorial analysis showed that HDL-C and INR were independent prognostic factors.2.The differences of CHOL,HDL-C,LDL-C and Apo A in the three groups of CTP were statistically significant,with grade A of CTP significantly higher than the other two groups.3.HDL-C and Apo A were significantly different in comparison between MELD score groups.4.The levels of CHOL,HDL-C,LDL-C and Apo A were positively correlated with the levels of ALB and CHE,and negatively correlated with PT,INR,CTP and MELD scores.5.HDL-C,CTP score,MELD score can predict the poor prognosis of patients with portal venous thrombosis in 2 years.Patients were at greatest risk of death when HDL-C < 0.82 mmol/L and MELD ? 11.04 minutes.Conclusions:1.High-density lipoprotein cholesterol as a new and simpler evaluation indicator,alone or in combination with the MELD score,can accurately assess the poor prognosis of patients with cirrhotic portal vein thrombosis.2.The levels of total cholesterol,high-density lipoprotein cholesterol,low-density lipoprotein cholesterol,and apolipoprotein A were significantly correlated with the severity of cirrhotic patients with portal vein thrombosis.
