Objective:This study through local musk after debridement and external Chinese medicine on diabetic foot(Wanger3),after comparing local debridement without dressing group in the control.Methods:According to the method of random control,52 patients who met the standard were divided into two groups,26 in the experimental group and 26 in the control group.The patients in the two groups were treated with basic medical treatment(control of blood glucose,blood pressure,blood lipid,infection,etc.)and then treated with surgery.After the wound preparation was completed,the control group(no dressing blank group)was covered with gauze and bandaged.The experimental group was treated with musk on the basis of the control group,then covered with gauze and bandaged.The drug was changed once a day for 4 weeks.In order to ensure the strict aseptic operation of the wound,the two groups of patients used aseptic transparent dressing to depict the contour of the wound,then used 0.1*0.1cm~2 transparent square paper to cover the aseptic transparent dressing to depict the contour of the wound,used a scanner to upload the image to the computer,used Image J image processor to calculate the wound area;at the same time Before treatment,at the 2nd and 4th week,the tip of tweezers was placed in the deepest part of the wound,the position was marked,and the depth of the wound was measured and recorded with a ruler;according to the local symptom integral table of diabetic foot wound,the integral points of the wound before treatment,the 2nd and the 4th week were recorded.All the recorded data were analyzed statistically,and the difference between the two groups was compared.Results:(1)The ankle brachial index of dorsalis pedis artery and the ankle brachial index of posterior tibial artery in the two groups increased(P<0.05),the area of the wound decreased(P<0.05),the depth of the wound(the depth of the sinus)became shallower(P<0.05),and the accumulation of local symptoms decreased(P<0.05).(2)At the second week of treatment,the reduction of wound area in the experimental group was better than that in the control group(P<0.05).There was no significant difference in ankle brachial index,wound depth and local symptom score between the experimental group and the control group(P>0.05).(3)After 4 weeks of treatment,the experimental group was superior to the control group in local symptom scores and wound area reduction(P<0.05),but there was no statistically significant difference in the degree of wound depth shallowness and the increase of ankle brachial index in the control group(P>0.05).(4)The total clinical effective rate was 100%in the experimental group and 96%in the control group.(5)In the experimental group,73%of the patients with effective treatment had 0<wound area?3cm~2.Conclusion:After local debridement,the local symptoms and signs of the wound of Wanger 3 diabetic foot can be significantly improved by external application or internal filling of musk,and the area and depth of the wound of Wanger 3 diabetic foot can be significantly reduced.After local debridement,the effect of external application or internal filling of musk is more obvious for the 0<wound area?3cm~2 Wanger 3diabetic foot.
